摘　　要：临床药师参与1例因创伤导致的罕见尖端赛多孢子菌皮肤软组织感染病例的药物治疗过程。临床药师在临床发挥了主观能动性,通过查阅多方循证证据,结合患者病情、药敏结果、肝肾功能等与医生共同为患者制定个体化用药方案,并进行药学监护、用药教育及出院后药学回访。保障了患者治疗用药的安全、有效、经济,提高了患者满意度。The clinical pharmacists participated in the drug treatment process of a rare case of soft tissue infection of Sardosporium apiospermum due to trauma.Clinical pharmacists played a subjective initiative in the clinic,reviewed evidence-based evidence from multiple sources,and combined the patient’s condition,drug sensitivity results,liver and kidney function,etc.,developed individualized medication plan for patient with doctors,and conducted pharmaceutical care,medication education,and post-discharge pharmacy followup visits to ensure the safety,effectiveness,and economy of treatment drugs,and improved the satisfaction of patients.
