摘  要:目的:探讨IL-35、TGF-β1与冠状动脉粥样硬化性心脏病(CHD)的相关性。方法:选取经冠状动脉造影明确诊断CHD的患者216例作为实验组,同期体检的表观健康人96例作为对照组。比较两组间转化生长因子-β1(TGF-β1)、IL-35、Hcy、LPa、HDL-C、TCH、TG、LDL-C、sdLDL-C的差异,分析IL-35、转化生长因子-β1(TGF-β1)与CHD和其影响因素的相关性,以及两者对CHD的诊断价值。实验组按CHD类型及病变累及血管数进行亚组分析,明确IL-35、TGF-β1与CHD严重程度之间的关系。结果:①CHD组较对照组IL-35升高(P﹤0.05),TGF-β1降低(P﹤0.05)。②单因素Logistic分析:CHD与IL-35呈正相关(β:0.194,OR:1.214,P﹤0.001),与TGF-β1呈负相关(β:-0.303,OR:0.738,P=0.003);校正混杂因素后仍表现为上述相关性(P﹤0.001)。分层分析显示CHD患者IL-35整体呈上升趋势(P﹤0.05),TGF-β1整体呈下降趋势(P﹤0.001)。③IL-35、TGF-β1、IL-35+TGF-β1对CHD均有预测价值(AUC:0.655,0.887,0.854 P﹤0.001)。④TGF-β1与所纳入的CHD影响因素均无关;而IL-35与Hcy呈正相关(Spearman系数:0.157 P=0.021)。⑤IL-35、TGF-β1与CHD严重程度无关。结论:IL-35升高和TGF-β1降低与CHD密切相关,并很少受其他因素影响。同时,TGF-β1降低可能是对CHD更好的独立预测因素。Objective:To investigate the correlation between IL-35,TGF-β1 and coronary heart disease(CHD).Methods:Patients(216 cases)with definite diagnosis of CHD using coronary angiography were selected as the experimental group,and 96 apparently healthy individuals examined at the same period were used as the control group.To compare the differences of TGF-β1,IL-35,Hcy,LPa,HDL-C,TCH,TG,LDL-C,and sdLDL-C between the two groups,and to analyze the correlation between IL-35,TGF-β1 and CHD and their influencing factors,as well as the diagnostic value of both for CHD.The experimental group was further analyzed by subgroup according to the type of CHD and the number of vessels involved in the lesion to clarify the relationship between IL-35,TGF-β1 and the severity of CHD.Results:①IL-35 was increased in the CHD group compared with the control group(P﹤0.05),TGF-β1 was decreased(P﹤0.05).②One-way logistic analysis:CHD was positively correlated with IL-35(β:0.194,OR:1.214,P﹤0.001)and negatively correlated with TGF-β(β:-0.303,OR:0.738,P=0.003).The above correlation was still observed after correction for confounding factors(P﹤0.001).Stratified analysis showed that IL-35 showed an overall increasing trend(P﹤0.05)and TGF-β1 showed an overall decreasing trend(P﹤0.001)in CHD patients.③IL-35,TGF-β1,IL-35+TGF-β1 had predictive value for CHD(AUC:0.655,0.887,0.854 P﹤0.001).④TGF-β1 was not associated with any of the included CHD influencing factors;while IL-35 was positively correlated with homocysteine(Spearman coefficient:0.157 P=0.021).⑤IL-35 and TGF-β1 were not associated with CHD severity.Conclusions:Elevated IL-35 and reduced TGF-β1 are strongly associated with CHD and are rarely influenced by other factors.Also,reduced TGF-β1 may be a better independent predictor of CHD.
