摘 要:目的:探探讨新诊断2型糖尿病(T2DM)患者血清脂肪细胞型脂肪酸结合蛋白/高分子量脂联素(A-FABP/HMWA)比值与24h血压变异性(BPV)的关系。方法:选取2020年1月至2021年8月,在海南省人民医院收治的新诊断T2DM患者150例作为研究对象,依据是否合并高血压分为单纯T2DM组91例和T2DM合并高血压组59例,另选取同期健康人群50例作为对照组。所有受试者均行24h动态血压监测和采用酶联免疫吸附试验测定血清A-FABP、HMWA水平,并计算24h收缩压变异性(SBPV)、24h舒张压变异性(DBPV)和A-FABP/HMWA比值。比较三组受试者一般资料、估算的肾小球滤过率(eGFR)、稳态模型胰岛素抵抗指数(HOMA-IR)、血脂、A-FABP/HMWA比值、24h SBPV及24h DBPV,并分析新诊断T2DM患者A-FABP/HMWA比值与24h SBPV、24h DBPV的相关性。结果:三组间HOMAIR、eGFR、TG、LDL-C、HDL-C、A-FABP/HMWA比值、24hSBPV、24hDBPV比较,差异具有统计学意义(P <0.05)。随着A-FABP/HMWA比值水平升高,新诊断T2DM患者的24hSBPV、24hDBPV水平依次升高(P <0.05)。新诊断T2DM患者的A-FABP/HMWA比值分别与24hSBPV、24hDBPV呈正相关(r分别为0.617、0.536,P <0.05)。多元线性逐步回归分析结果显示,在调整了其他相关因素后,A-FABP/HMWA比值分别与新诊断T2DM患者的24hSBPV、24hDBPV呈正相关(β值分别为0.425、0.315,P<0.05)。结论:新诊断T2DM患者A-FABP/HMWA比值与24h动态血压变异性呈正相关,A-FABP/HMWA比值升高是这类人群BPV升高的危险因素。Objective: To explore the relationship between serum adipocyte fatty acid binding protein/high molecular weight adiponectin(A-FABP/HMWA) ratio and blood pressure variability(BPV) in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods: A total of 150 patients with newly diagnosed T2DM treated in Hainan General Hospital from January 2020 to August 2021were selected as the research object, according to whether they were complicated with hypertension, the patients were divided into the simple T2DM group 91 cases and the T2DM with hypertension group 59 cases, and 50 healthy people in the same period were selected as the control group. All subjects underwent 24h ambulatory blood pressure monitoring and serum A-FABP and HMWA levels were detected by enzyme-linked immunosorbent assay, and 24h systolic blood pressure variability(SBPV), 24h diastolic blood pressure variability(DBPV) and A-FABP/HMWA ratio were calculated.The general data, estimated glomerular filtration rate(eGFR), homeostatic model insulin resistance index(HOMA-IR), lood lipid,dA-FABP/HMWA ratio, 24h SBPV and 24h DBPV among the three groups were compared, and the correlation between A-FABP/HMWA ratio and 24h SBPV and 24h DBPV in patients with newly diagnosed T2DM was analyzed. Results: There were significant differences in HOMA-IR, eGFR, TG, LDL-C, HDL-C, A-FABP/HMWA ratio, 24h SBPV and 24h DBPV among the three groups(P < 0.05). With the increase of A-FABP/HMWA ratio, the levels of 24h SBPV and 24h DBPV in patients with newly diagnosed T2DM increased in turn(P < 0.05). The A-FABP/HMWA ratio in patients with newly diagnosed T2DM was positively correlated with 24h SBPV and 24h DBPV, respectively(r= 0.617and 0.536(P < 0.05).Multiple linear stepwise regression analysis showed that after adjusting other relevant factors, the A-FABP/HMWA ratio was independently positively correlated with 24h SBPV and 24h DBPV in patients with newly diagnosed T2DM, respectively(β= 0.425 and 0.315(P < 0.05).
