摘　　要：目的利用非影像学内脏肥胖评估工具探讨一般人群内脏肥胖与甲状腺结节的相关性。方法以118019例健康体检成年人为研究对象,收集其人体测量学、生化检验和甲状腺超声检查的资料。内脏肥胖的评估采用内脏脂肪指数(VAI)、脂质蓄积指数(LAP)及心血管代谢指数(CMI)3个指标。利用logistic回归对3个内脏肥胖指标与甲状腺结节的相关性进行检验。结果对协变量进行校正后,仅有LAP与甲状腺结节显著正相关(OR=1.08,95%CI:1.04~1.13)。进一步对性别分组后发现在男性中内脏肥胖指标与甲状腺结节无相关性。而在女性中,VAI(OR=1.07,95%CI:1.00~1.13),LAP(OR=1.22,95%CI:1.14~1.31)和CMI(OR=1.07,95%CI:1.01~1.14)均显著与甲状腺结节正相关。结论内脏肥胖与甲状腺结节的正向关联性在女性中更为显著。Objective To investigate the correlation between the general population visceral obesity and thyroid nodules by using non-imaging visceral obesity assessment tools.Methods A total of 118019 adults with healthy physical examinations were selected as the research subjects,and data on anthropometrics,biochemical examinations,and thyroid ultrasound examinations were collected.The evaluation of visceral obesity uses three indicators:visceral fat index(VAI),lipid accumulation index(LAP)and cardiovascular metabolic index(CMI).Logistic regression was used to test the correlation between the three visceral obesity indicators and thyroid nodules.Results After adjusting for covariates,only LAP was significantly positively correlated with thyroid nodules(OR=1.08,95%CI:1.04-1.13).After further grouping by gender,it was found that there was nosignificant correlation between visceral obesity and thyroid nodules in men.In women,VAI(OR=1.07,95%CI:1.00-1.13),LAP(OR=1.22,95%CI:1.14-1.31)and CMI(OR=1.07,95%CI:1.01-1.14)were significantly positively associated with thyroid nodules.Conclusions The positively association between visceral obesity and thyroid nodules is more significant in women than in men.
