盐酸坦洛新缓释片联合会阴穴位药物注射治疗非手术意愿前列腺增生尿潴留的临床效果

摘　　要：目的:探讨盐酸坦洛新缓释片联合会阴穴位药物注射治疗非手术意愿前列腺增生尿潴留的临床效果。方法:选取2019年1月-2021年11月在青岛大学医学院松山医院泌尿外科门诊诊断为前列腺增生症合并尿潴留患者86例,按照随机数字表法分为A、B两组,各43例。A组完成41例,B组完成42例。A组给予盐酸坦洛新缓释片0.2 mg/d,每晚1次,连续口服30 d。B组给予盐酸坦洛新缓释片0.2 mg/d,每晚1次,连续口服30 d;分别于第8、15、22天在会阴穴位各注射利多卡因、新斯的明、曲安奈德的混悬液1次,共3次。治疗30 d后,观察两组治疗前后国际前列腺症状评分(IPSS评分)、生活质量评分(QOL评分)、最大尿流率(Qmax)、残余尿量变化及疗效。结果:治疗后,两组IPSS、QOL评分、残余尿量、Qmax均有不同程度降低或升高,但仅B组治疗后与治疗前比较,差异均有统计学意义(P<0.01);B组治疗后各指标均优于A组(P<0.01)。A组总有效率为26.83%,B组为88.10%,两组比较差异有统计学意义(P<0.01)。两组均无严重不良反应发生。结论:盐酸坦洛新缓释片联合会阴穴位药物注射治疗非手术意愿前列腺增生尿潴留有较好的临床疗效,可选择性应用于不愿意接受手术或不具备手术指征前列腺增生尿潴留的补充治疗。Objective:To investigate the clinical effect of Tamsulosin Hydrochloride Sustained-release Tablets combined with perineal acupoint drug injection in the treatment of urinary retention in prostatic hyperplasia with nonoperative willingness.Method:A total of 86 cases of urinary retention with benign prostatic hyperplasia in Urology Clinic,Songshan Hospital,Qingdao University Medical College from January 2019 to November 2021 were selected and randomly divided into group A and group B,43 cases in each group.A total of 41 cases in group A and 42 cases in group B were completed.Group A was given Tamsulosin Hydrochloride Sustained-release Tablet 0.2 mg/d,qn,for consecutive 30 d,group B was given Tamsulosin Hydrochloride Sustained-release Tablets 0.2 mg/d,qn,for consecutive 30 d;Lidocaine,Neostigmine and Triamcinolone Acetonide suspension were injected at the perineal acupoints on the 8th,15th and 22nd,respectively,for 3 times.After 30 d of treatment,the changes of international prostate symptom score(IPSS score),quality of life score(QOL score),maximum urine flow rate(Qmax)and residual urine volume before and after treatment,the effects in group A and B were observed.Result:After treatment,the IPSS score,QOL score,residual urine volume and Qmax in two groups were decreased or increased,but only group B had significant differences after treatment compared with those before treatment(P<0.01);after treatment,all indexes in group B were better than those in group A(P<0.01).The total effective rate of group A was 26.83%,group B was 88.10%,and there was significant difference between the two groups(P<0.01).No serious adverse reactions occurred in two groups.Conclusion:Tamsulosin Hydrochloride Sustained-release Tablets combined with perineal acupoint drug injection in the treatment of urinary retention in nonoperative prostatic hyperplasia has good clinical efficacy,it can be selectively applied to the complementary treatment of urinary retention in patients who are unwilling to undergo surgery or do not have surgical indicat
