骨水泥型加长柄人工髋关节置换术治疗高龄股骨粗隆间骨折的临床效果

摘　　要：目的:探讨股骨粗隆间骨折高龄患者采取骨水泥型加长柄人工髋关节置换术治疗的临床效果。方法:选择2019年6月-2021年11月于扬州友好医院接受治疗的股骨粗隆间骨折高龄患者66例,根据随机奇偶抽签结果划分观察组(33例,奇数签)与对照组(33例,偶数签),对照组接受股骨近端防旋髓内钉(PFNA)内固定治疗,观察组采取骨水泥型加长柄人工髋关节置换术治疗。观察比较两组治疗总有效率,测定两组手术前后髋关节活动角度变化,记录对比两组术后并发症总发生率。结果:观察组治疗总有效率明显高于对照组(P<0.05),术后观察组髋关节屈曲、内收、外展、外旋活动角度大于对照组(P<0.05)。术后观察组并发症总发生率低于对照组(P<0.05)。结论:股骨粗隆间骨折高龄患者通过采取骨水泥型加长柄人工髋关节置换术治疗具有确切效果,能够促进患者髋关节活动功能恢复,安全性理想,提倡运用推广。Objective:To investigate the clinical effect of bone cement-type lengthened stem artificial hip replacement in elderly patients with intertrochanteric fracture of femur.Method:A total of 66 elderly patients with intertrochanteric fracture of femur who were treated in Yangzhou Friendship Hospital from June 2019 to November 2021 were selected,and according to the result of random draw parity divided into observation group (33 cases,odd number sign) and control group (33 cases,even number sign).The control group received proximal femoral intramedullary nail antirotation (PFNA) prevention internal fixation treatment,the observation group was treated with bone cement-type lengthened stem artificial hip replacement.The total effective rate of treatment was observed and compared,the changes of hip joint angle before and after treatment were measured,and the total incidence of complications after treatment were recorded and compared between the two groups.Result:The total effective rate in the observation group was significantly higher than that in the control group (P<0.05).After operation,the activity angles of hip flexion,adduction,abduction and external rotation in the observation group were greater than those in the control group (P<0.05).After operation,the total incidence of complications in the observation group was lower than that in the control group (P<0.05).Conclusion:Bone cement-type lengthened stem artificial hip replacement for elderly patients with intertrochanteric fracture of femur has definite effect and can promote the recovery of hip mobility and function,with ideal safety,which should be advocated for application and promotion.
