摘 要:目的:探讨应用血管介入术治疗颅内动脉瘤的临床效果。方法:选择2020年5月-2021年5月武汉市汉口医院收治的颅内动脉瘤患者108例作为本次研究的对象,依据随机数字表法将入选患者分为血管组及夹闭组。夹闭组(54例)行颅内动脉瘤夹闭术治疗,血管组(54例)行血管介入术治疗。比较两组的疗效、并发症发生率,比较两组术前、术后3个月核转录因子κB(NF-κB)及基质金属蛋白酶9(MMP-9)水平。结果:与夹闭组相比,血管组的手术时间、住院时间更短,术中出血量更少,并发症发生率更低,差异均有统计学意义(P<0.05)。术前,两组NF-κB、MMP-9水平比较,差异无统计学意义(P>0.05);术后3个月,两组NF-κB、MMP-9水平较术前呈现显著降低趋势,且血管组的上述指标均低于夹闭组,差异有统计学意义(P<0.05)。结论:血管介入术对机体创伤少,患者手术用时和住院时间短,是治疗颅内动脉瘤的理想手段,有助于促进MMP-9等指标的恢复,术后并发症少,临床应用具有较高的安全性。Objective:To explore the clinical effect of vascular intervention in the treatment of intracranial aneurysm.Method:A total of 108 patients with intracranial aneurysms treated in Hankou Hospital of Wuhan City from May 2020 to May 2021 were selected as the subjects of this inquiry,and the selected patients were divided into vascular group and clipping group according to the random number table method.The clipping group(54 cases)was treated with clipping intracranial aneurysm,and the vascular group(54 cases)was treated with vascular intervention.The efficacy and complication incidence between the two groups were compared,the nuclear factorκB(NF-κB)and matrix metalloproteinase-9(MMP-9)levels before and 3 months after operation between the two groups were compared.Result:Compared with the clipping group,shorter surgical time and hospitalization time,less intraoperative bleeding and lower complications occurred in the vascular group,and the differences were statistically significant(P<0.05).Before operation,the NF-κB and MMP-9 levels in the two groups were no significant differences(P>0.05).Three months after operation,the levels of NF-κB and MMP-9 showed a significant downward trend compared with those before operation,and the vascular group were lower than those in the clipping group,and the differences were statistically significant(P<0.05).Conclusion:Vascular intervention is less trauma for the body and short surgical time and hospitalization.It is an ideal treatment for intracranial aneurysm,which is helpful to promote the repair of MMP-9 and other indicators,with fewer postoperative complications and high safety in clinical application.
