摘　　要：目的通过对1例使用伏立康唑产生异常兴奋反应的患者进行分析,研究发生不良反应的原因并及时调整给药方案。方法临床药师利用血药浓度监测作为技术手段,医药指南及文献作为信息支持,患者实际疾病状态为基础,为临床医师提供参考意见,优化治疗方案。结果医生采纳临床药师的建议,对治疗方案进行了调整,不良反应症状得到缓解,患者用药依从性得到提高。结论面对复杂多变的病情,不能只简单的依据说明书用药,需要结合患者的生理和病理情况制定个体化用药方案。Objective To study the causes of adverse reactions and adjust the administration plan in time by analyzing a patient with abnormal excitatory reaction induced by voriconazole.Methods Clinical pharmacists use blood concentration monitoring of therapeutic drugs as a technical means,medical guidelines and literature as information support,based on the actual disease status of patients,to provide reference for clinicians and improve the treatment plan.Results The doctor adopted the clinical pharmacist′s suggestion to change the treatment plan,the adverse reaction symptoms were relieved and the compliance of patients was improved.Conclusion In the face of complex and changeable conditions,it is not only necessary to use drugs according to the instructions,but also need to develop individualized medication plan according to the physiological and pathological conditions of patients.
