摘　　要：目的:弥散峰度成像(DKI)及神经突方向离散度与密度成像(NODDI)技术探索帕金森病(PD)患者脑灰质核团微结构的变化及其与疾病进展的关系。方法:选择24例PD患者并分为早期(Hoehn-Yahr分级Ⅰ~Ⅱ级,10例)和中晚期PD组(Hoehn-YahrⅢ~Ⅳ级,14例),同期选择16名年龄及性别相匹配的健康志愿者作为对照进行DKI及NODDI,测量双侧黑质、红核、壳核、丘脑、苍白球及尾状核的参数值,并取平均值。采用独立样本t检验比较PD组和对照组间参数值差异;采用单因素方差分析比较早期、中晚期PD患者和正常对照组间参数值的差异;采用Spearman相关分析临床Hoehn-Yahr分级与参数值的相关性。结果:PD患者黑质及壳核的神经突内体积分数(Vic)、神经突方向离散度(ODI)值低于健康对照组(P<0.05);对照组、早期和中晚期PD患者双侧黑质及壳核Vic、ODI值呈递减趋势,中晚期PD组低于对照组(P<0.05)。PD患者Hoehn-Yahr分级与黑质及壳核的Vic、ODI值呈负相关(P<0.05)。结论:黑质和壳核中的Vic和ODI值有助于PD患者的早期诊断,并可能有助于确定疾病严重程度及评估疾病进展。Purpose:To explore the microstructure changes of brain gray matter nuclei in patients with Parkinson’s disease and its relationship with disease progression by using diffusion kurtosis imaging(DKI)and neurite orientation dispersion and density imaging(NODDI).Methods:Twenty-four PD patients were divided into early stage(Hoehn-YahrⅠ-Ⅱstage,10 cases)and mid-late PD group(Hoehn-YahrⅢ-Ⅳstage,14 cases).Age and gender matched healthy volunteers were also recruited in this study.Both the PD group and the control group were undergone with DKI and NODDI imaging to measure the parameters values of the bilateral substantia nigra,red nucleus,thalamic nucleus,putamen,globus pallidus and caudate nucleus,and the average values of them were calculated.The independent sample t-test was used to compare the differences in the parameter values of the nucleus between the PD group and the control group;the one-way analysis of variance was used to compare the differences in the parameter values of the nucleus in the early,mid-late stages PD patients and the normal control group;the Spearman correlation analysis was used to analyze the correlation between Hoehn-Yahr staging and the parameter values of each nucleus.Results:The intracellular volume fraction(Vic)and Orientation dispersion index(ODI)values of the SN and putamen of PD patients were lower than those of the control group(P<0.05);There was a decreasing trend of the Vic,ODI values of the bilateral SN and putamen in the control group,early and mid-late stage PD,the Vic,ODI values in mid-late stage PD were lower than those in control group(P<0.05).The Hoehn-Yahr staging of PD patients was negatively correlated with the Vic and ODI values of the substantia nigra and putamen(P<0.05).Conclusion:The values of Vic and ODI in the substantia nigra and putamen are helpful for the early diagnosis of PD patients,and may help determine the severity of the disease and evaluate the disease progression.
