摘  要:目的:探讨C臂锥形束CT不同成像时间脑血管成像效果及临床应用价值。方法:用C臂锥形束CT在头部体模上进行扫描。采集时间分别为5s、8s和20s,分别用图像噪声、信噪比(SNR)和对比噪声比(CNR)客观评价图像质量。计算剂量面乘积(DAP)和有效剂量(ED)来评估辐射剂量。回顾性分析137例行C臂锥形束CT检查或治疗的脑血管病变患者的影像学及临床资料对其进行主观评价。结合二维DSA造影、CT、MRI检查,分析C臂锥形束CT不同成像时间对病变的检出及显示效果,并分析其临床应用价值。结果:20s C臂锥形束CT成像的噪声明显低于5s和8s成像。20s的信噪比和CNR明显高于8s和5s C臂锥形束CT成像。C臂锥形束CT5s DSA模式可用于动脉瘤、动静脉畸形、脑肿瘤供血动脉及脑血管狭窄的诊断和治疗。8s DSA模型适用于颅内静脉的检测和治疗。20s DR模式可用于颅内出血病灶及支架的显示。结论:C臂锥形束CT不同成像时间的临床应用范围不同,合理选择成像时间将有利于合理的诊疗案的制订。Purpose: To investigate the effect and clinical value of C-arm cone beam CT angiography with different imaging time. Methods: The head phantom was scanned with C-arm cone beam CT. The acquisition time was 5 s, 8 s and 20 s respectively. The image quality including image noise, signal-to-noise ratio(SNR) and contrast to noise ratio(CNR) was objectively evaluated. The dose area product(DAP) and effective dose(ED) were calculated to evaluate the radiation dose. The imaging and clinical data of 137 patients with cerebrovascular diseases who underwent C-arm cone beam CT examination or treatment were retrospectively analyzed. Combined with twodimensional DSA, CT and MRI, the detection and display effect of C-arm cone beam CT with different imaging time were analyzed, and its clinical application value was analyzed.Results: The noise of CBCT with 20 s acquisition time was significantly lower than that of CBCT with 5 s and 8 s acquisition time. The SNR and CNR of CBCT with 20 s acquisition time were significantly higher than that of CBCT with 8 s and 5 s acquisition time. C-arm cone beam CT with 5 s DSA mode could be used for the diagnosis and treatment of aneurysms, arteriovenous malformations,cerebral tumor feeding arteries and cerebral vascular stenosis. The 8 s DSA model was suitable for the detection and treatment of intracranial veins. 20 s DR mode could be used to display intracranial hemorrhage lesions and stents.Conclusion: The clinical application range of C-arm cone beam CT with different imaging time is different.Reasonable selection of imaging time will be conducive to the formulation of reasonable diagnosis and treatment cases.
