摘 要:目的:使用宽体探测器高分辨率CT对冠状动脉支架进行成像,并进行4种不同重建方式的重建,评估经皮冠状动脉介入治疗(PCI)术后患者腔内支架的可视化程度。方法:对连续的37例PCI术后患者,使用宽体探测器高分辨率CT行冠状动脉CT血管成像(CTA),采用高清扫描模式,并用4种不同重建算法进行图像重建,包括标准(stnd)算法,细节(detail)算法、高清标准(HD-stnd)和高清细节(HD-detail)算法,对植入支架血管进行图像后处理,获得血管拉直图像以及相应部位的血管垂直横断面图像,对图像进行客观评价(包括支架后冠脉内腔直径、噪声、晕状伪影)和主观图像质量评分(Likert4级评分法),结果采用重复测量方差分析进行比较,P<0.05为差异具有统计学意义。结果:使用HD-detail和HD-stnd重建算法图像的测量支架后冠脉内腔直径均大于其他重建算法,HD-detail和HD-stnd重建算法图像的晕状伪影均小于其他重建算法,HD-detail重建算法图像的噪声最大,HD-detail和HD-stnd重建算法主观图像质量评分高于其他重建算法,差异具有统计学意义(P<0.05)。结论:采用宽体探测器高分辨率CT结合高分辨率扫描模式对冠状动脉支架进行成像,并使用高清重建(HD-detail和HD-stnd)算法,能够提高冠状动脉支架内管腔情况的可视化程度,为PCI术后患者的随访观察提供更加精准的依据。Purpose:To evaluate the visualization of coronary stents with four different reconstruction algorithms in the whole-organ high-definition CT scanner.Methods:Thirty-seven patients after stenting underwent coronary computed tomography angiography(CTA)with high-definition scanning mode in a whole-organ high-definition CT scanner.Four different reconstruction algorithms(stnd,detail,HD-stnd,HD-detail)were applied to reconstruct the images of the stented coronary artery.The objective image quality score included the intrastent luminal diameter,noise and blooming artifact,and the Likert 4 point scale was performed to evaluate subjective image quality score.Differences of objective and subjective parameters of different reconstruction algorithms were compared by repeated measurement variance analysis,P-value<0.05 was considered statistically significant.Results:The intrastent luminal diameter using HD-detail and HD-stnd was larger than the other reconstruction algorithms.The blooming artifact of the images using HD-detail and HD-stnd were smaller than that of other reconstruction algorithms.The noise of the images using HD-detail was the largest.The subjective image quality score using HD-detail and HD-stnd was larger than the other reconstruction algorithms.The difference was with statistical significance(P<0.05).Conclusions:The whole-organ high-definition CT scanner can be used to improve the visualization of coronary stents by the highdefinition scanning mode with HD-detail and HD-stnd reconstruction algorithms,and provide more accurate basis for follow-up observation of patients after PCI.
