308 nm准分子光联合钙泊三醇倍他米松软膏治疗寻常型白癜风临床观察

摘　　要：目的:探究308 nm准分子光联合钙泊三醇倍他米松软膏治疗寻常型白癜风疗效及对外周血Th1/Th2的影响。方法:以2019年1月-2021年1月117例寻常型白癜风患者为研究对象,采用随机数字表法分为对照A组、对照B组和观察组,39例/组,对照A组给予钙泊三醇倍他米松软膏治疗,对照B组给予308 nm准分子光治疗,观察组给予308 nm准分子光联合钙泊三醇倍他米松软膏治疗。比较三组疗效、治疗期间不良反应发生率、临床症状变化情况、生活质量指数(Dermatology life quality index,DLQI)及外周血干扰素-γ(Interferon-γ,IFN-γ)、白介素-4(IL-4)及白介素-10(IL-10)水平、白斑面积氧化应激指标[超氧化物歧化酶(Superoxide dismutase,SOD)、丙二醛(Malondialdehyde,MDA)、谷胱甘肽过氧化物酶(Glutathione peroxidase,GSH-Px)]变化情况。结果:观察组治疗总有效率高于对照组A组、对照B组(P<0.05);观察组白斑消退时间均短于对照组A组、对照B组(P<0.05);观察组治疗后白斑面积小于对照组A组、对照B组,DLQI评分低于对照组A组、对照B组(P<0.05);观察组治疗后IFN-γ、IL-4及IL-10水平低于对照组A组、B组(P<0.05);观察组治疗后GSH-Px、SOD水平高于对照组A组、对照B组,MDA水平低于对照组A组、对照B组(P<0.05);两组不良反应发生率对比无明显差异(P>0.05)。结论:308 nm准分子光联合钙泊三醇倍他米松软膏治疗寻常型白癜风的临床疗效较好,可明显缩小白斑面积,缩短临床症状消失时间,且能促进Th1/Th2平衡,减轻皮损部位氧化应激反应,并能改善患者生活质量。Objective To explore the effect of 308nm excimer laser combined with calcipotriol betamethasone ointment in the treatment of vitiligo vulgaris and its influences on peripheral blood Th1/Th2.Methods A total of 117 patients with vitiligo vulgaris were enrolled as the research objects between January 2019 and January 2021.They were divided into control A group,control B group and observation group by random number table method,39 cases in each group.The control A group was treated with calcipotriol betamethasone ointment,control B group was treated with 308 nm excimer laser,and observation group was treated with 308 nm excimer laser and calcipotriol betamethasone ointment.The curative effect,the incidence of adverse reactions during treatment,the changes of disappearance of clinical symptoms,lesion range,the scores of dermatology life quality index(DLQI),levels of peripheral blood interferon-γ(IFN-γ),interleukin-4(IL-4),interleukin-10(IL-10),and oxidative stress indexes at white spot area[superoxide dismutase(SOD),malondialdehyde(MDA),glutathione peroxidase(GSH-Px)]before and after treatment in the three groups were observed.Results The total response rate of treatment in observation group was higher than that in control A group and control B group(P<0.05).The disappearance time of vitiligo in observation group was shorter than that in control A group and control B group(P<0.05).After treatment,white spot area in observation group was smaller than that in control A group and control B group,and DLQI score was lower than that in control A group and control B group(P<0.05).After treatment,levels of IFN-γ,IL-4 and IL-10 in observation group were lower than those in control A group and control B group(P<0.05),GSH-Px and SOD levels were higher than those in control A group and control B group,and MDA level was lower than that in control A group and control B group(P<0.05).There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion The clinical curative effect of
