外用重组牛碱性成纤维细胞生长因子凝胶联合2940nm点阵铒激光治疗凹陷性痤疮瘢痕的研究

摘　　要：目的:探究外用重组牛碱性成纤维细胞生长因子(rb-bFGF)凝胶联合2940 nm点阵铒激光在凹陷性痤疮瘢痕中的应用效果及可能机制。方法:选取2018年6月-2020年10月笔者医院凹陷性痤疮瘢痕患者112例,随机分为观察组、对照组,各56例。对照组给予2940 nm点阵铒激光治疗,观察组在对照组基础上联合外用rb-bFGF凝胶治疗。比较两组临床疗效、症状恢复时间、不良反应、治疗前后温哥华瘢痕量表(Vancouverscar scale,VSS)、痤疮瘢痕权重评分(E’chelle d~,évaluation clinique des cicatrices d~,acné,ECCA)、血清细胞因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-17(IL-17)、白细胞介素-10(IL-10)]、皮肤生理指标[角质层含水量、经皮水分丢失(TEWL)、皮肤红斑情况(a值)]水平。结果:经治疗,观察组总有效率为94.64%,较对照组的75.00%高(P<0.05);经治疗,观察组水肿时间、红斑持续时间、结痂时间均较对照组短(P<0.05);两组治疗后VSS、ECCA评分均较本组治疗前降低,且观察组低于对照组(P<0.05);两组治疗后血清TNF-α、IL-17、IL-10水平均较本组治疗前降低,且观察组低于对照组(P<0.05);观察组治疗后角质层含水量较本组治疗前提高,且高于对照组,TEWL、a值均较本组治疗前降低,且低于对照组(P<0.05);两组治疗期间不良反应发生率相比,差异无统计学意义(P>0.05)。结论:凹陷性痤疮瘢痕是痤疮后常见瘢痕状态,外用rbFGF凝胶能有效辅助2940 nm点阵铒激光治疗瘢痕,克服治疗过程中患者皮损的加重,改善皮肤屏障功能。Objective To explore the effect and possible mechanism of topical recombinant bovine basic fibroblast growth factor(rb-bFGF)gel combined with 2940 nm fractional erbium laser in depressed acne scars.Methods A total of 112 patients with depressed acne scars in the hospital from June 2018 to October 2020 were selected and randomly divided into an observation group and a control group,with 56 cases in each group.The control group was treated with 2940nm fractional erbium laser.The observation group was treated with external rb-bFGF gel on the basis of the control group.Comparison of clinical efficacy,symptom recovery time,adverse reactions,Vancouver Scar Scale(VSS)before and after treatment,acne scar weight score(ECCA),serum cytokines[tumor necrosis factor-α(TNF-α),interleukin-17(IL-17),interleukin-10(IL-10)],skin physiological indicators[stratum corneum water content,transdermal water loss(TEWL),skin erythema(a value)]level.Results After treatment,the total effective rate of the observation group was 94.64%,which was higher than the 75.00%of the control group(P<0.05).After treatment,the observation group’s edema time,erythema duration,and scab time were shorter than those of the control group(P<0.05).the VSS and ECCA scores of the two groups after treatment were lower than those before the treatment,and the observation group was lower than the control group(P<0.05).Serum TNF-α,IL-17,IL-10 levels after treatment in the two groups were lower than those before treatment in this group,and the observation group was lower than the control group(P<0.05).The stratum corneum content of the observation group after treatment The water volume was higher than this group before treatment,and higher than the control group,TEWL,a values were lower than this group before treatment,and lower than the control group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups during the treatment period Statistical significance(P>0.05).Conclusion Depressed acne scars are common scars after acne.Exte
