摘 要:目的:探讨负压创面治疗技术(Negative pressure wound therapy,NPWT)联合重组人碱性成纤维细胞生长因子(Recombinant human basic fibroblast growth factor,rh-bFGF)持续冲洗治疗慢性创面的临床效果。方法:选取医院烧伤整形外科2020年1月-12月收治的100例慢性创面患者为研究对象,随机分为A组和B组,每组各50例。A组采取NPWT联用生理盐水冲洗治疗,B组为NPWT联用rh-bFGF溶液持续冲洗。比较两组肉芽组织覆盖率、疼痛评分、创面细菌阳性率及炎症指标。结果:治疗后,两组疼痛评分、白细胞计数、C反应蛋白均降低,且B组低于A组,差异均有统计学意义(P<0.05)。B组新生肉芽组织覆盖率和创面愈合率明显高于A组,创面细菌阳性率低于A组,差异均有统计学意义(P<0.05)。结论:NPWT联用rh-bFGF持续冲洗可降低创面感染率,促进一期肉芽组织新生,减少二期手术间隔时间,有一定临床应用价值。Objective To observe the clinical effect of NPWT combined with rh-bFGF continuous flushing in the treatment of chronic wounds.Methods A total of 100 patients with chronic wounds treated in the department of burn and plastic surgery of our hospital from January to December 2020 were randomly divided into the group A(n=50)and the group B(n=50).The group A was treated with NPWT combined with normal saline,and the group B was treated with NPWT combined with rh-bFGF.The coverage rate of granulation tissue,pain score,bacteria positive rate and inflammation indexes of wound were compared between the two groups.Results After treatment,the pain score,WBC and C-reactive protein of the two groups decreased,and the group B were lower than the group A,the differences were statistically significant(P<0.05).The coverage rate of new granulation tissue and wound healing rate in the group B were significantly higher than those in the group A(P<0.05).The positive rate of wound bacteria in the group B was lower than that in the group A(P<0.05).Conclusion NPWT combined with rh-bFGF can reduce wound infection rate,promote granulation regeneration of primary wound,and reduce the interval between secondary operations,which has certain clinical value.
