中药瘢痕膏辅助腹部带蒂皮瓣治疗手部深度烧伤疗效分析

摘　　要：目的:分析中药瘢痕膏辅助腹部带蒂皮瓣治疗手部深度烧伤的临床疗效。方法:选择80例手部深度烧伤患者为研究对象,对照组(40例)采用腹部皮瓣带蒂术治疗,观察组(40例)采用中药瘢痕膏辅助腹部皮瓣带蒂术治疗,观察两组临床疗效,对比两组治疗前后的手指总主动活动度(Total active movement,TAM)、上肢功能指数(The upper extremity functional index,UEFI)及生活质量改善情况,采用视觉模拟评分法(Visual analogue scale,VAS)评价两组疼痛程度,记录两组住院时间、创面愈合时间及不良反应情况。结果:治疗后,观察组总有效率、TAM情况、UEFI、生活质量各项评分均显著高于对照组,差异有统计学意义(P<0.05)。观察组VAS评分、住院时间及创面愈合时间均显著低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率与对照组相比,差异无统计学意义(P>0.05)。结论:中药瘢痕膏辅助腹部带蒂皮瓣治疗手部深度烧伤疗效显著,可有效减少患者痛苦,提升生活质量。Objective To analyze the clinical effect of traditional Chinese medicine scar ointment assisted with abdominal pedicled skin flap in the treatment of deep hand burns.Methods Eighty patients with deep hand burns were selected as the research object.The control group(40 cases)was treated withabdominal pedicled skin flap,and the observation group(40 cases)was treated with traditional Chinese medicine scar ointment assisted with abdominal pedicled skin flap.The clinical efficacy of the two groups was observed.The improvement of TAM,UEFI and quality of life of the two groups were compared before and after treatment.The pain degree of the two groups was evaluated by VAS,and the hospitalization time,wound healing time and adverse reactions of the two groups were recorded.Results After treatment,the total effective rate,TAM,UEFI and quality of life scores in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The VAS score,length of stay and wound healing time in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion The clinical effect of abdominal pedicled skin flap assisted by traditional Chinese medicine scar ointment in the treatment of deep hand burn is remarkable,which can effectively reduce pain and improve the quality of life.
