摘　　要：目的:研究不同形式的鼻整形修复术对于鼻背皮肤菲薄就医者的效果对比。方法:选择2017年8月-2019年6月开滦总医院林西医院收治的50例鼻背部皮肤菲薄鼻整形就医者,随机分为乙烯组与软骨组,乙烯组(25例)采用膨体聚四氟乙烯联合自体真皮方案,软骨组(25例)采用自体肋软骨联合自体筋膜方案,比较两组的术后恢复情况、美学指标、心理状态、不良反应发生率以及就医者满意度。结果:术后软骨组创口愈合时间和肿胀时间均少于乙烯组(P<0.05);两组术后鼻子美学相关指标均比术前有所提高,且软骨组高度、长度、鼻面角均高于乙烯组(P<0.05);术后两组焦虑自评量表(Self-Rating Anxiety Scale,SAS)与抑郁自评量表(Self-Rating Depression Scale,SDS)评分情况均好于术前,且软骨组SAS、SDS评分均低于乙烯组(P<0.05);软骨组术后不良反应发生率(12.00%)低于乙烯组(40.00%,P<0.05);软骨组就医者对于鼻部皮肤流畅程度、弹性以及鼻子流畅度的满意度(76.00%,92.00%,88.00%)均高于乙烯组(48.00%,60.00%,64.00%,P<0.05)。结论:自体软骨联合自体筋膜术式术后恢复时间更短,就医者的美学效果和临床满意度高,就医者焦虑和抑郁情绪较轻,临床安全性高。Objective To compare the effect of different rhinoplasty and repair for patients with thin nasal dorsum skin.Methods Fifty patients with thin skin on the back of the nose treated in Linxi Hospital of Kailuan General Hospital from August 2017 to June 2019 were selected.The ethylene group(25 cases)was treated with expanded polytetrafluoroethylene combined with autologous dermis,and the cartilage group(25 cases)was treated with autologous costal cartilage combined with autologous fascia.The postoperative recovery,preoperative and postoperative nose aesthetic indexes,psychological status,incidence of adverse reactions and patient satisfaction were compared between the two groups.Results The wound healing time and swelling time in cartilage group were shorter than those in ethylene group(P<0.05).The indexes of nasal aesthetics in both groups were improved after surgery,and the height,length and nasal Angle in cartilage group were higher than those in ethylene group(P<0.05).The self-rating Anxiety Scale(SAS)and self-rating Depression Scale(SDS)scores of the two groups were better after operation than before,and the SAS and SDS scores of the cartilage group were lower than those of the ethylene group(P<0.05).The incidence of postoperative adverse reactions in cartilage group(12.00%)was lower than that in ethylene group(40.00%,P<0.05).The degree of nasal skin smoothness,elasticity and nasal smoothness in cartilage group(76.00%,92.00%,88.00%)were higher than those in ethylene group(48.00%,60.00%,64.00%,P<0.05).Conclusion The postoperative recovery time of autologous cartilage combined with autologous fascia surgery is shorter,the aesthetic effect and clinical satisfaction of patients are high,the anxiety and depression of patients are lighter,and the clinical safety is higher.
