基于“轻身”药物分析中药治疗儿童肥胖症

摘　　要：目的:整理古代文献记载的“轻身”药物,并挖掘中国期刊全文数据库(CNKI)及相关文献书籍中治疗儿童肥胖的处方规律,分析“轻身”药物在治疗儿童肥胖方剂中的应用规律及可能的作用机制。方法:挖掘CNKI及相关文献中治疗儿童肥胖的方剂90首,录入中医传承辅助平台(V2.5),通过药频统计、组方规律、新方分析进行数据整理。并结合“轻身”药物进行数据分析。结果:在治疗儿童肥胖方剂的151味中药里,“轻身”药物共47味,占31.1%;“轻身”药物使用频次较高的有茯苓、白术、泽泻等;四气以温、寒、平为主,五味以甘、苦、酸为主;方剂中“轻身”药物归经频次从高到低为肝经、肾经、脾经、胃经、肺经等;证候分析以脾虚湿阻为主;运用关联规则及复杂系统熵聚类方法分析得到常用药物组合7个、核心药物组合12个及潜在新方6个。结论:“轻身”药物在儿童肥胖治疗中使用较为广泛,其中高频药物的作用功效与儿童肥胖的常见证型相对应;药物归经提示肝、肾在儿童肥胖的发病机制和处方用药的重要性,为临床治疗和预防儿童肥胖提供新的思路和方法。Objective:Through summarizing the“light body”medicinals recorded in ancient literature,and mining the Chinese Journal Full-text Database(CNKI)and related literature and books to get the prescription rules for the treatment of childhood obesity,and to analyze the application rules and possible mechanism of“light-body”medicinals in the formulas for childhood obesity.Methods:Ninety formulas for childhood obesity from CNKI and other literature and books were mined and input into Traditional Chinese Medicine Inheritance Support System(V2.5).Combined with the“light-body”medicinals,data were analyzed through medicinal frequency statistics,prescription rules and new formula analysis.Results:Among the 151 Chinese medicinals for the treatment of childhood obesity,there were 47“light-body”medicinals,accounting for31.1%of total medicinals.“Light body”medicinals of high frequency were Fuling(Poria),Baizhu(Rhizoma Atractylodis Macrocephalae),Zexie(Rhizoma Alismatis).The four properties usually involved warm,cold and neutral,and five flavors mainly involved sweet,bitter and sour.In formulas,“light body”medicinals usually entered liver meridian,kidney meridian,spleen meridian,stomach meridian,lung meridian with the frequency from high to low.According to syndrome analysis,spleen deficiency and dampness accumulation is the major one.There were 7 commonly used medicinal combinations,12 core medicinal combinations and 6 potential new formulas obtained by using association rules and complex system entropy clustering.Conclusions:“Light body”medicines are widely used in the treatment of childhood obesity,and the efficacy of high-frequency drugs corresponds to the common syndrome types of childhood obesity.The meridians of drugs indicate the importance of liver and kidney in the pathogenesis of childhood obesity and prescription medicines,and provide new ideas and methods for clinical treatment and prevention of childhood obesity.
