清热养阴活血方治疗牙周脓肿临床疗效及对龈沟液IL-1β、IL-6水平的影响

摘　　要：目的:观察清热养阴活血方治疗青少年牙周脓肿的功效及对患者龈沟液白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)的影响。方法:140例青少年牙周脓肿患者按随机数字表法分为对照组和观察组各70例。对照组局部脓肿切开引流后予甲硝唑口服,观察组局部脓肿切开引流后予清热养阴活血方治疗。两组均治疗7 d(1个疗程)后,观察疗效。比较两组临床疗效、西医症状积分、中医证候积分及龈沟液中的IL-1β、IL-6水平和复发率。结果:观察组临床疗效显著优于对照组,观察组治疗后西医症状积分、中医证候积分及龈沟液内IL-1β、IL-6水平与治疗前和对照组治疗后比较均显著降低,差异有统计学意义(P<0.05)。观察组复发率也明显低于对照组。结论:清热养阴活血方治疗青少年牙周脓肿能改善机体炎症状态,促进症状改善,提高临床疗效。Objective:To observe the efficacy of Qingre Yangyin Huoxue Formula(清热养阴活血方)on interleukin(IL)-1βand IL-6 in gingival sulcus fluid of adolescent patients with periodontal abscess.Methods:One hundred and forty adolescent patients with periodontal abscess were divided into control group and observation group according to random number table method,70 patients in each group.The control group was given oral administration of metronidazole after incision and drainage of local abscess,while the observation group was given Qingre Yangyin Huoxue Formula after incision and drainage of local abscess.After both groups were treated for 7 days(1 treatment course),the therapeutic effect was observed.The clinical effectiveness,symptom score,traditional Chinese medicine(TCM)syndrome score,levels of IL-1βand IL-6 in gingival sulcus fluid levels,and recurrence rate were compared between the two groups.Results:The clinical efficacy of the observation group was significantly better than that of the control group.After treatment,symptom scores,TCM syndrome scores and the levels of IL-1βand IL-6 in gingival sulcus fluid in the observation group were significantly lower than those before treatment and those after treatment in the control group,with statistically significant differences(P<0.05).The recurrence rate also decreased significantly in observation group.Conclusion:Qingre Yangyin Huoxue Formula could relieve the inflammation of the body in the treatment of periodontal abscess in adolescent patients,promote the improvement of symptoms and significantly improve clinical effectiveness.
