激光包皮环切术与一次性包皮吻合器治疗包皮过长、包茎的疗效比较

摘　　要：目的探讨一次性包皮吻合器与激光包皮环切术治疗包皮过长、包茎的临床疗效,为临床包皮环切术的选择提供依据。方法回顾性分析2018年1月至2019年3月我院接受手术治疗的包皮过长、包茎患者200例,按治疗方法不同分为对照组和观察组各100例。对照组接受一次性包皮吻合器手术治疗,观察组接受激光包皮环切术治疗,观察两组并发症发生情况,通过VAS评分观察比较两组术中、术后疼痛情况,观察比较两组临床指标:手术费用、手术时间,评估两组患者术后伤口满意度情况。结果对照组并发症发生率为12.00%,稍高于观察组的10.00%,数据比较,差异无统计学意义(P>0.05);观察组术中、术后各时间段疼痛评分(VAS)与对照组比较,差异均无统计学意义(P>0.05);观察组手术费用低于对照组,手术时间长于对照组,差异有统计学意义(P<0.05);两组患者术后对伤口外观满意度比较,差异无统计学意义(P>0.05)。结论一次性包皮吻合器手术具有手术时间短、外观平整的优点,但激光包皮环切术手术费用低,术后并发症少,且适应于各类包皮过长、包茎及不同年龄阶段患者,包皮手术应根据患者具体情况进行适当选择。Objective To explore the clinical curative effect of disposable prepuce stapler and laser circumcision in the treatment of redundant prepuce and phimosis,so as to provide basis for the selection of clinical circumcision.Methods We selected 200 patients with redundant prepuce and phimosis receiving treatment in our hospitalfrom January 2018 to March 2019,and divided them into control group and observation group according to the parity number of admission order,with 100 cases in each group.The patients in the control group were treated with disposable prepuce stapler,whereasthe patients in the observation group were treated with laser circumcision,and the complications in the two groups were observed.The intraoperative and postoperative pain of the two groups were observed and compared using VAS score.We also observed and compared the clinical indexes of the two groups including operation cost,operation time,and patients’satisfaction with the postoperative wound.Results The incidence of complications in the control group was 12.00%,which was slightly higher than the 10.00%in the observation group,and the difference was not statistically significant(P>0.05).There was no statistically significant difference in intraoperative and postoperative pain score(VAS)between the two groups(P>0.05).The operation cost in the observation group was significantly lower than that in the control group,and the operation time was significantly longer than that in the control group(P<0.05).There was no significant difference in satisfaction with postoperative wound appearance(P>0.05).Conclusion The disposable prepuce stapler operation has the advantages of short operation time and satisfactory wound appearance,while the laser circumcision has the advantages of low operation cost,less postoperativecomplications,and is suitable for all kinds of patients with redundant prepuce or phimosis at different age.Therefore,circumcision operation should be selected according to the specific conditions of patients.
