稳心颗粒联合血栓心脉宁对老年隐匿性冠心病疗效及心率变异性的影响

摘　　要：目的探讨稳心颗粒联合血栓心脉宁对老年隐匿性冠心病(silent myocardial ischemia,SMI)临床疗效、预后及心率变异(HRV)的影响。方法选择门诊或住院诊治的老年隐匿性冠心病患者107例,在二级预防(阿司匹林、他汀类药物)基础上,随机分为两组,对照组53例给予曲美他嗪等西药治疗,研究组54例服用稳心颗粒联合血栓心脉宁,治疗12周后对两组患者心肌缺血改善及心率变异情况进行观察和比较,同时收集一年内患者主要不良心血管事件(MACE)。结果研究组心电图疗效(有效率72.22%)优于对照组(有效率50.94%),P<0.05。研究组SDNN、SDANN、rMSSD、PNN50、高频功率(HF)均升高,低频功率(LF)、低频和高频功率之比(LF/HF)下降,P<0.05。研究组SDNN、SDANN、rMSSD、LF、HF、LF/HF改善情况优于对照组,P<0.05。研究组MACE(9.26%)低于对照组(24.53%),P<0.05。结论稳心颗粒联合血栓心脉宁可以提高老年隐匿性冠心病疗效,有效改善心率变异,减少MACE,且不良反应小,值得临床上推广应用。Objective To investigate the clinical efficacy,prognosis and effect of Wenxin granules combined with Xueshuan Xinmai Ning on senile asymptomatic myocardial ischemia(SMI)and HRV.Methods A total of 107 senile coronary heart disease sick persons with asymptomatic myocardial ischemia were randomly divided into two groups on the basis of secondary prevention(aspirin,statins.The control group(n=53)was treated with trimetazidine and other western medicine,while the study group(n=54)was treated with Wenxin granules combined with Xueshuan Xinmai Nin.After 12 weeks of treatment,the improvement of myocardial ischemia and heart rate variation were observed and compared between the two groups,and major adverse cardiovascular events(MACE)were collected in one year.Results The electrocardiogram efficacy of the study group(effective rate 72.22%)was superior to that of the control group(effective rate 50.94%),P<0.05.SDNN,SDANN,RMSSD,HF and PNN50 were all increased in the study group,while low spectral power(LF)and low frequency/high spectral power ratio(LF/HF)were decreased,P<0.05.The improvement of SDNN,SDANN,RMSSD,LF,HF and LF/HF in the study group was better than that in the control group,P<0.05.Cardiovascular events in the study group(9.26%)were lower than those in the control group(24.53%),P<0.05.Conclusion Wenxin granules combined with Xueshuan Xinmai Ning can improve the efficacy of senile asymptomatic myocardial ischemia,effectively improve heart rate variation,reduce MACE,and have small untoward effect,which deserves extensive clinical application.
