经膜肺吸入七氟烷在体外循环中的应用

摘  要:目的探讨经膜肺吸入七氟烷在体外循环中的应用。方法收集我院2021年3月至8月6例使用了经膜肺吸入七氟烷瓣膜病患者的基本情况,记录这6例患者在体外循环开始后、复温后、体外循环结束前静脉血气,评价其对膜肺功能的影响和患者的预后。结果6例患者术经颈内静脉以较低剂量泵入丙泊酚和右美托咪定,联合经膜肺吸入七氟烷均达到满意的麻醉效果,BIS值稳定在40左右,七氟烷吸入浓度维持在0~5%;七氟烷没有影响膜肺的功能,术后呼吸机时间为(17.2±2.2)h,ICU停留时间为(37.8±18.5)h,没有出现相关并发症。结论经膜肺吸入七氟烷在体外循环中使用可以降低术中丙泊酚等静脉麻醉药的使用量,不会影响膜肺的功能。经膜肺吸入七氟烷在体外循环中应用是安全的。Objective To explore the application of sevoflurane inhalation through membrane lung in cardiopulmonary bypass.Methods The basic information of 6 patients with valvular disease treated with sevoflurane inhalation through membrane lung from March 2021 to August 2021 was collected.The venous blood gas of these 6 patients was recorded after the start of cardiopulmonary bypass,after rewarming,and before the end of cardiopulmonary bypass,and their effects on membrane lung function and prognosis were evaluated.Results During the operation,propofol and dexmedetomidine were pumped into the internal jugular vein at a lower dose,and sevoflurane was inhaled through the membrane lung,which achieved satisfactory anesthetic effect.The BIS value was stable at about 40,and the sevoflurane inhalation concentration was maintained at 0~5%;Sevoflurane did not affect the function of membrane lung.The postoperative ventilator time was(17.2±2.2)h,and the ICU stay time was(37.8±18.5)h.There were no related complications.Conclusion sevoflurane inhalation through membranous lung during cardiopulmonary bypass can reduce the use of propofol and other intravenous anesthetics during operation,it will not affect the function of membrane lung,Sevoflurane inhalation through membrane lung is safe in cardiopulmonary bypass.
