小剂量硝普钠和多巴胺持续泵入在顽固性心力衰竭治疗中的应用探讨

摘　　要：目的分析小剂量硝普钠和多巴胺持续泵入在顽固性心力衰竭治疗中的应用效果。方法选取2019年9月—2020年11月敦化市医院收治的顽固性心力衰竭患者94例,通过随机数字表法分为对照组47例,为呋塞米片、螺内酯、贝那普利、地高辛等基础性治疗,观察组47例为基础性治疗的基础上小剂量硝普钠和多巴胺持续泵入治疗,对比两组疗效、心功能及用药安全性。结果观察组有效率97.87%(46/47),高于对照组的80.85%(38/47),差异有统计学意义(P<0.05)。治疗前两组患者心功能指标差异无统计学意义(P>0.05);治疗后,观察组心功能各指标表现优于对照组,差异有统计学意义(P<0.05)。观察组患者不良反应和对照组比,差异无统计学意义(P>0.05)。结论小剂量硝普钠、多巴胺持续泵入可有效提升患者治疗总有效率,改善心功能,无严重不良反应率。Objective To analyze the application effect of small dose sodium nitroprusside and dopamine continuous pumping in the treatment of refractory heart failure.Methods A total of 94 patients with refractory heart failure who were treated in Dunhua City Hospital from September 2019 to November 2020 were selected and randomly divided into the control group of 47 cases,including furosemide tablets,spironolactone,benazepril,and digoxin.Other basic treatment,47 patients in the observation group were treated with low-dose sodium nitroprusside and dopamine continuous infusion based on basic treatment,and the efficacy,cardiac function and drug safety were compared between the two groups.Results The effective rate of the observation group was 97.87%(46/47),which was higher than that of the control group 80.85%(38/47),and the difference was statistically significant(P<0.05).There was no significant difference in cardiac function between the two groups before treatment(P>0.05);after treatment,the indexes of cardiac function in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in adverse reactions between the observation group and the control group(P>0.05).Conclusion Small dose of sodium nitroprusside and dopamine continuous pumping can effectively improve the total effective rate of patients,improve heart function,without serious adverse reaction rate.
