抗癌消痛汤联合耳穴压豆治疗中重度癌性疼痛临床观察

摘　　要：目的观察自拟抗癌消痛汤联合耳穴压豆治疗中重度癌性疼痛的临床效果。方法选取中重度癌性疼痛患者76例,采用随机数字表法将其分为2组,各38例,对照组采用盐酸羟考酮缓释片治疗,治疗组在对照组的基础上采用自拟抗癌消痛汤联合耳穴压豆治疗。2组均治疗14 d。比较2组的卡氏功能状态(KPS),体力状况(PS),疼痛程度数字分级(NRS)评分,爆发痛出现次数,阵痛起效、持续时间,盐酸羟考酮缓释片日均用量,不良反应发生率。结果2组患者治疗后卡氏功能状态、体力状况、疼痛程度数字分级评分较治疗前均有改善(P<0.05),且治疗组优于对照组(P<0.05);治疗组患者爆发痛出现次数少于对照组(P<0.05);治疗组患者镇痛起效与持续时间均优于对照组(P<0.05);治疗组患者盐酸羟考酮缓释片日均用量少于对照组(P<0.05);治疗组患者总有效率为81.58%(31/38),高于对照组的60.53%(23/38)(P<0.05);治疗组患者不良反应总发生率为28.95%(11/38),低于对照组的63.16%(24/38)(P<0.05)。结论自拟抗癌消痛汤联合耳穴压豆治疗中重度癌性疼痛,能够快速持久地发挥镇痛效果,减轻疼痛程度,降低盐酸羟考酮缓释片日均用量及药物不良反应发生率,提高临床效果。Objective To observe the clinical effect of Kangai Xiaotong decoction combined with auricular point pressing in the treatment of moderate to severe cancer pain.Methods 76 patients with moderate to severe cancer pain were selected and divided into 2 groups by random number table method,with 38 cases in each group.The control group was treated with oxycodone hydrochloride sustained-release tablets,and the treatment group was treated with self-made Kangai Xiaotong decoction combined with auricular point pressing on the basis of the control group.Both groups were treated for 14 days.Carsten function status(KPS),physical status(PS),digital rating of pain(NRS),outbreak times,onset of labor pain,duration,daily dose of oxycodone hydrochloride sustained release tablets and incidence of adverse reactions were compared between the two groups.Results The KPS,PS and NRS scores of the two groups of patients after treatment were improved compared with those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).The frequency of pain outburst in treatment group was less than that in control group(P<0.05).The efficacy and duration of analgesia in treatment group were better than those in control group(P<0.05).The daily dose of oxycodone hydrochloride sustained release tablets in the treatment group was less than that in the control group(P<0.05).The total effective rate in the treatment group was 81.58%(31/38),which was higher than that in control group(60.53%,23/38)(P<0.05).The incidence of adverse reactions in treatment group was 28.95%(11/38),which was lower than that in control group(63.16%,24/38)(P<0.05).Conclusion Self-made Kangai Xiaotong decoction combined with auricular point pressing in the treatment of moderate to severe cancer pain can quickly and permanently exert analgesic effect,reduce the degree of pain,reduce the daily average dosage of oxycodone hydrochloride sustained release tablets and the incidence of adverse drug reactions,and improve the clinical effect.
