川芎龙蛭汤联合耳穴电探测贴压治疗脑梗死气虚血瘀证临床研究

摘　　要：目的:探讨川芎龙蛭汤联合耳穴电探测贴压治疗脑梗死气虚血瘀证疗效。方法:选取脑梗死气虚血瘀证患者82例,以随机数字表法分为西医组、联合组,每组41例。西医组予以常规西医治疗,联合组在西医组治疗基础上予以川芎龙蛭汤联合耳穴电探测贴压治疗,两组均连续治疗2周。比较两组治疗前后美国国立卫生院卒中量表(NIHSS)评分、主要中医证候积分、血液流变学指标、炎症指标。比较两组治疗效果、预后情况。结果:治疗2周后,联合组总有效率为92.68%,高于西医组的73.17%(P<0.05)。治疗2周后,联合组主要中医证候积分、NIHSS评分低于西医组,差异有统计学意义(均P<0.05)。治疗2周后,联合组PV、HCT、ESR、hs-CRP、TNF-α、PCT水平低于西医组,差异有统计学意义(均P<0.05)。治疗后3个月,联合组良好率56.10%,高于西医组的34.15%(P<0.05)。结论:川芎龙蛭汤联合耳穴电探测贴压可改善脑梗死气虚血瘀证微循环,减轻炎症反应,促进神经功能重建。Objective:To explore curative effect of Chuanxiong Longzhi decoction combined with auricular point electrical detection and sticking pressure on cerebral infarction with Qixu Xueyu syndrome.Methods:82 patients with cerebral infarction of Qixu Xueyu syndrome were divided into western medicine group and combined group by random number table method,41 cases in each group.The western medicine group was treated with conventional western medicine,and the combined group was treated with Chuanxiong Longzhi decoction combined with auricular point electrical detection and sticking pressure on the basis of western medicine group,both groups were treated for 2 weeks.NIHSS score,main TCM syndrome scores,blood rheology indexes and inflammatory response-related indexes were compared between the two groups before and after treatment.The treatment effect and prognosis of the two groups were counted.Results:After 2 weeks of treatment,the total effective rate in the combined group was 92.68%higher than 73.17%in the western medicine group(P<0.05).After 2 weeks of treatment,the scores of main TCM syndromes and NIHSS in the combined group were lower than those in the western medicine group,difference statistically significant(all P<0.05).After 2 weeks of treatment,the levels of PV,HCT,ESR,hs-CRP,TNF-α,PCT in the combined group were lower than those in the western medicine group,difference statistically significant(all P<0.05).3 months after treatment,the good prognosis rate in the combined group was 56.10%higher than 34.15%in the western medicine group(P<0.05).Conclusion:Chuanxiong Longzhi decoction combined with auricular electroprobe and sticking pressure can improve the microcirculation of cerebral infarction patients with Qixu Xueyu syndrome,reduce inflammatory response,promote the reconstruction of nerve function.
