亚甲蓝联合曲安奈德与利多卡因在产后骶髂关节致密性骨炎的效果

摘　　要：目的观察亚甲蓝联合曲安奈德与利多卡因在产后骶髂关节致密性骨炎骶髂关节腔内注射的治疗效果,为该病的临床治疗提供可参考的方案。方法选取滨州医学院附属医院疼痛科诊断为产后骶髂关节致密性骨炎的患者60例,随机分为观察组与对照组,每组30例;观察组患者骶髂关节腔内注射亚甲蓝联合曲安奈德与利多卡因,对照组患者骶髂关节腔内注射曲安奈德联合利多卡因,分别于治疗后6小时、1、6个月观察患者的疼痛数字评分(NRS评分)、Harris髋关节功能评分、骶髂关节激惹征阳性率,并与对照组相比较。结果与治疗前相比,观察组患者治疗后6小时、1、6个月的NRS评分降低、Harris髋关节功能评分升高、骶髂关节激惹征阳性率下降,临床症状减轻,关节功能提升,P<0.05;与对照组相比较,观察组患者在治疗后6个月时的NRS评分与骶髂关节激惹征阳性率均降低,Harris髋关节功能评分提高,疗效维持优于对照组,P<0.05。结论亚甲蓝联合曲安奈德与利多卡因骶髂关节腔内注射治疗产后骶髂关节致密性骨炎疗效好,疗效维持时间长,可在临床推广使用。Objective To observe the effect of methylene blue combined with triamcinolone acetonide and lidocaine in sacroiliac joint cavity injection of postpartum osteitis condensans ilii and to provide reference treatment for clinical practice.Methods Sixty patients with postpartum osteitis condensans ilii diagnosed in the Pain Department of Binzhou Medical University Hospital were randomly divided into the observation group(n=30)and the control group(n=30)according to the double-blind method.The observation group was injected with methylene blue combined with triamcinolone acetonide and lidocaine into the sacroiliac joint cavity,and the control group was injected with triamcinolone acetonide combined with lidocaine into the sacroiliac joint cavity.The pain numerical score(NRS score),Harris hip joint function score and the positive rate of sacroiliac joint irritation sign were observed at 6 hours,1 month and 6 months after the treatment,and the results were compared with those of the control group.Results Compared with those before the treatment,the NRS score and the positive rate of sacroiliac joint irritation decreased,the Harris hip function score increased,the clinical symptoms relieved and joint function improved in the observation group at 6 hours,1 month and 6 months after the treatment,with statistically significant differences(P<0.05).Compared with those in the control group,after 6 months of the treatment,the NRS score and the positive rate of sacroiliac joint irritation syndrome in the observation group were lower than those in the control group,the Harris hip function score and the curative effect in the observation group was higher than the those in the control group,with statistical difference(P<0.05).Conclusion Intraarticular injection of methylene blue combined with triamcinolone acetonide and lidocaine in the treatment of postpartum osteitis condensans ilii has good efficacy and long duration of efficacy,so it can be widely used in clinical practice.
