重型颅脑创伤救治新进展

摘　　要：重型颅脑创伤致残、致死率高,其成功救治一直是难题。国内外科学家围绕这些临床救治难题持续发力、摸索前进,在基础和临床方面进行了大量的研究,攻克了众多难关,在此基础上,以严格的循证医学标准搜集新的研究结果作为证据,发布了系列的重型颅脑创伤治疗指南。2016年美国脑创伤基金会发布的《重型颅脑创伤治疗指南(第四版)》是最近的一版,对重型颅脑创伤救治提出了更为精准的诊断、治疗推荐。本文针对第四版指南涉及的部分重要问题,评述该指南发布后国内外有关颅脑创伤诊治的最新动态,以期进一步提高诊治水平。Severe traumatic brain injury(sTBI)leads to high morbidity and mortality,and its successful treatment has always been tough.Scientists at home and abroad continue to work hard to search for the keys to these clinical problems.They have conducted a large number of basic and clinical studies,and have tried to overcome a great number of difficulties.On this basis,new research results have been collected based on strict evidence-based medical standards to publish a series of guidelines for the treatment of sTBI.The Guidelines for the Management of Severe Traumatic Brain Injury(Fourth Edition)issued by the American Brain Trauma Foundation in 2016 is the latest edition,which provides more accurate diagnosis and treatment recommendations for the treatment of sTBI.This article focuses on some important issues involved in the fourth edition of the guidelines,and reviews the latest developments in the diagnosis and treatment of sTBI at home and abroad after the publication of the guidelines with a view to further improving diagnosis and treatment.
