摘  要:目的探讨应用常规超声仪检测眼动脉(OA)、视网膜中央动脉(CRA)及睫状后动脉(PCA)的血流动力学变化,以明确不同环境下其血流动力学的变化规律以及对视网膜的影响。方法将18只新西兰大白兔分为3组,D0组(常温组)、D4组(低温4 h组)、D8组(低温8 h组),在-25℃环境下建立低温模型,使用超声仪测量兔球后血流情况及OA、CRA及PCA的血流参数,包括收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、搏动指数(PI)、阻力指数(RI)、收缩期舒张期血流速度比(S/D)、加速时间(AT)、减速时间(DT),测量结束后将视网膜剥离,并做免疫组化检测,观察各层细胞的变化。结果各组实验兔OA、CRA及PCA彩色多普勒超声结果显示:与D0组相比,D8组球后血流信号明显减少,与D0组相比,D4组兔的OA、CRA及PCA的PI、RI及S/D增大、AT,DT延长,差异有统计学意义(P<0.05);D8组兔OA、CRA及PCA的PSV、EDV减低,PI、RI及S/D增大、AT,DT延长,差异有统计学意义(P<0.05)。而D8组与D4组相比,D8组兔OA、CRA及PCA的PSV、EDV减低,PI、RI、S/D增大,差异有统计学意义(P<0.05),免疫组化:D8组INL层细胞损伤较多,激活了caspase-3的高表达。结论常规超声检查可以更为敏感地反映低温环境下OA、CRA、PCA的血流动力学改变,为临床早期诊治视网膜疾病提供指导。Objective To determine the hemodynamic changes of ophthalmic artery(OA),central retinal artery(CRA)and posterior ciliary artery(PCA)in different environments and their effects on the retina by using conventional ultrasound.Methods Eighteen new zealand white rabbits were divided into normal temperature group(control group D0 group),low temperature 4 h group(D4 group),and low temperature 8 h group(D8 group)and established the rabbit model of low temperature in-25℃environment.Ultrasound instrument was used to measure retrobulbar flow conditions of the room temperature and low temperature rabbit and the blood flow parameters of OA,CRA and PCA,including systolic peak flow velocity(PSV),end-diastolic velocity(EDV),pulse index(PI),resistance index(RI),diastolic systolic blood flow velocity ratio(S/D),acceleration time(AT),and deceleration time(DT).When the measurements were completed,retinal detachment and immunohistochemical detection were performed to observe the changes of cells in each layer.Results The results of OA,CRA and PCA color doppler ultrasound in each group showed that,compared with the D0 group,the retrobulbar blood flow signal was significantly reduced in D8 group.Compared with D0 group,PI,RI and S/D of OA,CRA and PCA in D4 group were increased,AT and DT were prolonged,the difference was statistically significant(P<0.05).In the D8 group,the PSV and EDV of OA,CRA and PCA were decreased,PI,RI and S/D were increased,AT and DT were prolonged and the difference was statistically significant(P<0.05);Compared with the D4 group,the PSV and EDV of OA,CRA and PCA in the D8 group were decreased,PI,RI,S/D were increased,the difference was statistically significant ( P <0.05). Immunohistochemistry showed the cell damage of INL layer in D8 group was more, which activated the high expression of caspase-3. Conclusion Conventional ultrasound can more sensitively reflect the hemodynamic changes of OA, CRA and PCA under low temperature environment, and provide guidance for the early diagnosis and treatment of retinal dis
