摘  要:目的:探讨≤34周早产儿静脉营养相关性胆汁淤积的发生与血脂、血清脂联素的相关性。方法:以我院2021年1—12月期间收治的85例≤34周早产儿为研究对象,患儿入院后第1、2、3、4、5周分别检测其血脂、血清脂联素水平,以发生静脉营养相关性胆汁淤积为事件终点,根据是否发生静脉营养相关性胆汁淤积分为发生组和未发生组,比较两组早产儿一般资料以及血脂、血清脂联素水平。采用Cox回归分析模型分析血脂、血清脂联素与静脉营养相关性胆汁淤积发生的相关性。结果:85例早产儿中,34例发生静脉营养相关性胆汁淤积,发生率为40.00%;发生组总胆汁酸水平高于未发生组(P<0.05);以85例早产儿为因变量,建立Cox回归风险模型,Cox回归分析血清脂联素(HR=0.794)是静脉营养相关性胆汁淤积发生的保护因素,总胆汁酸是静脉营养相关性胆汁淤积发生的危险因素(HR=1.016)。结论:总胆汁酸是≤34周早产儿静脉营养相关性胆汁淤积发生的危险因素,血清脂联素是保护因素。Objective:To explore the correlation between the occurrence of venous nutrition-related cholestasis and blood lipid and serum adiponectin in premature infants at≤34 weeks.Methods:85 premature infants of≤34 weeks in our hospital from January 2021 to December 2021 were taken as the research object.Their blood lipid and serum adiponectin levels were detected at the 1 st,2 nd,3 rd,4 th and 5 th week after admission.The event endpoint was the occurrence of venous nutrition-related cholestasis,which was divided into the occurrence group and the non-occurrence group according to whether there was venous nutrition-related cholestasis or not.The general information,blood lipid and serum adiponectin levels of the two groups were compared.Cox regression analysis model was used to analyze the correlation between blood lipid,serum adiponectin and the occurrence of venous nutrition-related cholestasis.Results:Among the 85 premature infants,34 cases developed intravenous nutrition-related cholestasis,with an incidence of 40.00%.The level of total bile acid in occurrence group was higher than that in non-occurrence group(P<0.05).Taking 85 premature infants as dependent variables,a Cox regression risk model was established.By Cox regression analysis,serum adiponectin(HR=0.794)was the protective factor for the occurrence of intravenous nutrition-related cholestasis,and total bile acid was the risk factor for the occurrence of intravenous nutrition-related cholestasis(HR=1.016).Conclusion:Total bile acid is a risk factor of venous nutrition-related cholestasis in premature infants≤34 weeks old,and serum adiponectin is a protective factor.
