摘 要:目的:分析在直径≤2 cm乳腺小结节良恶性鉴别诊断中应用3.0T乳腺动态增强MRI(DCE-MRI)联合钼靶X线的价值。方法:选择南京市高淳人民医院2020年3月—2021年11月收治的直径≤2 cm乳腺小结节患者120例为研究对象,均给予DCE-MRI检查与钼靶X线检查,以病理检查结果为金标准,分析DCE-MRI检查情况,并比较DCE-MRI检查、钼靶X线检查及二者联合检查的准确度、灵敏度、特异度。结果:在DCE-MRI边缘、内部强化形态学特征、时间-信号强度曲线分型、早期增强率方面,良性组检出率明显比恶性组低,差异有统计学意义(P<0.05)。在Slope、SlopeR方面,良性组数值明显比恶性组低,差异有统计学意义(P<0.05)。在诊断准确度、灵敏度、特异度方面,DCE-MRI检查以及二者联合检查数值明显高于钼靶X线(P<0.05);DCE-MRI检查与二者联合检查比较差异无统计学意义(P>0.05)。结论:在直径≤2 cm乳腺小结节良恶性鉴别诊断中,DCE-MRI检查的诊断价值优于钼靶X线。为了进一步提高诊断准确度,可加强二者的联合应用,以此为临床治疗提供指导依据。Objective To study and analyze the clinical value of 3.0T dynamic enhanced breast MRI(DCE-MRI)combined with molybdenum target X-ray in differential diagnosis of benign and malignant breast nodules with diameter≤2 cm.Methods A total of 120 patients with small breast nodules≤2 cm in diameter admitted to our hospital from March 2020 to November 2021 were selected as the research objects.All patients were given DCE-MRI and molybdenum target X-ray examination,and the DCE-MRI examination was analyzed with pathological examination as the gold standard.The accuracy,sensitivity and specificity of DCE-MRI,molybdenum target X-ray and their combination were compared.Results In terms of DCE-MRI edge,morphological characteristics of internal enhancement,time-signal intensity curve typing and early enhancement rate,the detection rate of benign group was significantly lower than that of malignant group(P<0.05).In Slope and SlopeR,the value of benign group was significantly lower than that of malignant group(P<0.05).In terms of diagnostic accuracy,sensitivity and specificity,the values of DCE-MRI and their combination were significantly higher than those of molybdenum target X-ray(P<0.05).There was no significant difference between DCE-MRI and combined MRI(P>0.05).Conclusion In the differential diagnosis of benign and malignant small breast nodules with≤2 cm in diameter,DCE-MRI examination has a very high application value and is better than molybdenum target X-ray.In order to further improve the diagnostic accuracy,the combined application of the two can be strengthened,so as to provide guidance for clinical treatment.
