摘 要:目的:探讨锥形束CT(CBCT)在上颌埋伏阻生尖牙诊断和治疗中的应用效果。方法:选择2018年10月—2021年8月在广西国际壮医医院就诊的上颌埋伏阻生尖牙青少年患者31例作为研究对象,均行CBCT检查,分析上颌骨中埋伏尖牙的位置,与邻近切牙的位置关系,观察邻牙牙根吸收情况。结果:全景片中阻生尖牙位于2~4区时切牙牙根吸收发生率高于0~1区,差异有统计学意义(P<0.05)。CBCT矢状面图像中17颗上颌阻生尖牙(45.4%)处于唇侧位置,13颗(34.1%)处于腭侧位置,9颗(20.5%)处于颌骨中央。13颗(33.3%)阻生尖牙造成邻近切牙牙根吸收;其中腭侧阻生尖牙造成的切牙牙根吸收的发生率高于唇侧、颌骨中央。结论:上颌埋伏尖牙常见于唇侧,位于腭侧的阻生尖牙更容易造成邻近切牙的牙根吸收,尖牙越靠近近中越易导致邻近切牙牙根吸收。CBCT能够在三维方向显示上颌埋伏尖牙的空间位置以及对邻牙造成的根吸收情况,为临床诊断和治疗方案的制定提供可靠依据。Objective To investigate the application of cone beam computed tomography(CBCT)in the diagnosis and treatment of maxillary impacted canines.Methods From October 2018 to August 2021,31 adolescent cases of maxillary impacted canine patients who were treated in Guangxi International Zhuang Medical Hospital were selected as the research objects.CBCT was performed to analyze the maxillary impact the position of the canine and the adjacent incisor,observe the root resorption of the adjacent incisors.Results In the panoramic film,the incidence of root resorption of the incisor was higher when the impacted canine is located in zone 2~4 than that in zone 0~1,and the difference was statistically significant(P<0.05).In the CBCT sagittal image,17 maxillary impacted canines(45.4%)were in the labial position,13(34.1%)were in the palatine position,and 9(20.5%)were in the center of the jawbone.Thirteen(33.3%)impacted canines caused root resorption of adjacent incisors;the incidence of root resorption of incisors caused by palatal impacted canines was higher than that of labial and central jaws.Conclusion Maxillary impacted canines often occur on the labial side.Impacted canines on the palatal side is likely to cause root resorption of adjacent incisors.The closer the canine is to the mesial,the more likely it is to absorb the root of the adjacent incisor.CBCT can display the spatial position of the maxillary impacted canine and the root resorption in three dimensions,providing a reliable basis for clinical diagnosis and the formulation of treatment plans.
