摘 要:目的 阐明广东省东莞市结核分枝杆菌氨基糖苷类耐药相关基因的突变特征,为东莞市结核病的有效防控提供科学依据。方法 207株结核分枝杆菌临床分离株(其中66株链霉素耐药,15株卷曲霉素耐药,2株丁胺卡那霉素耐药)行核酸质谱分析检测氨基糖苷类耐药相关基因(rpsL、rrs、eis基因)点突变。结果59株耐氨基糖苷结核菌检出rpsL或/和rrs基因突变,其中49株携有rrs基因突变,20株携有rpsL基因突变,未检出eis基因突变。124株对氨基糖苷类药物敏感的结核分枝杆菌测出57例rrs G1484T突变。rrs A1401G、rpsL Lys43Arg (AAG→AGG)、Lys88Arg AAG→AGG等突变与耐药表型高度一致。结论 rrs和rpsl基因突变是结核分枝杆菌耐氨基糖苷类药物的重要因素。Objective To characterize the molecular mutation in genes associated with aminoglycoside re-sistance in Mycobacterium tuberculosis in Guangdong,and to provide scientific evidence for effective prevention and control of tuberculosis in this province.Methods A total of 207 clinical isolates of M.tuberculosis(66strepto-mycin-resistant,15 capreomycin-resistant,and 2 amikacin-resistant) were analyzed by mass spectrometric analysis of nucleic acids to determine the point mutations in aminoglycoside resistance-associated genes(rpsL,rrs,and eis).Results Mutations in rpsL and/or rrs genes were detected in 59 aminoglycoside-resistant tuberculosis isolates,of which,49 carried rrs mutation,20 carried rpsL mutation,and none carried eis mutation.Of 124 amino-glycoside-sensitive M.tuberculosis strains,57 were found to carry G1484T mutation of rrs gene.Mutations such as rrs gene A1401G,rpsL gene Lys43Arg(AAG→AGG),and Lys88Arg AAG→AGG were highly concordant with the resis-tance phenotype.Conclusion Mutations in rrs and rpsL genes are important culprit for aminoglycoside resistance in M.tuberculosis.
