不同免疫诱导方案对肾移植患者的临床疗效观察

摘　　要：目的 观察3种不同免疫诱导方案的肾移植术临床有效性及安全性评价。方法 回顾性分析2015—2020年在我院实施肾移植的235例患者,根据术前不同免疫诱导方案分为3组,兔抗人胸腺细胞免疫球蛋白(ATG)组、巴利昔单抗组、巴利昔单抗联合抗人T淋巴细胞兔免疫球蛋白(rATG-F)组,比较术后移植肾功能恢复情况,术后发生急性排斥、骨髓抑制及感染等指标。结果 3组患者在移植术后不同时间点的移植肾功能恢复差异无统计学意义(P>0.05),但3组患者术后急性排斥反应的发生率不同,差异具有统计学意义(P<0.05),巴利昔单抗联合r ATG-F发生急性排斥反应发生率最低(5.6%),巴利昔单抗组发生急性排斥反应发生率最高(26.1%)。3组患者术后移植肾功能延迟恢复(DGF)、消化道反应、骨髓抑制及感染发生率差异均无统计学意义(P>0.05)。结论 3种免疫诱导方案术后发生急性排斥反应率不同,未发现rATG-F与ATG、巴利昔单抗对移植肾功能恢复情况、DGF、消化道反应、骨髓抑制和感染发生率存在差异。Objective To investigate the clinical efficacy and safety profiles with three immune induction regimens in kidney transplantation.Methods A retrospective analysis was performed on 235 patients who underwent kidney transplantation in our hospital between 2015 and 2020.The subjects were assigned to three groups according to the preoperative immune induction regimens:antithymocyte globulin(ATG) group,basiliximab group,and basiliximab plus r ATG-Fresenius(rATG-F) group.Three groups were compared for postoperative recovery of the renal graft function,onset of acute rejection,bone marrow suppression,and infection.Results There was no significant difference in recovery of the renal graft function across the three groups at different time points post-transplant(P>0.05).The incidence rate of postoperative acute rejection differed significantly among the three groups(P0.05).Conclusion Three immune induction regimens result in varied rates of acute rejection post-transplant.There is no difference among r ATG-F,ATG and basiliximab in relation topost-transplant recovery of renal graft function,DGF,digestive tract reaction,bone marrow suppression and infection.
