摘　　要：目的 分析不同预后的爆震性耳聋(ED)患者畸变产物耳声发射(DPOAE)检查结果。方法 选取本院2020年1月-2022年1月收治的66例ED患者作为研究对象,根据不同预后情况将患者分为A组(治愈,n=14)、B组(显效,n=27)和C组(无效,n=25),比较三组患者DPOAE检查结果。结果 66例患者(132耳)均行纯音听阈测试与DPOAE测试,132耳中共37耳存在纯音听阈异常,52耳存在DPOAE异常,DPOAE测试听力异常检出率39.39%显著高于纯音听阈测试的28.03%(χ^(2)=4.316,P=0.038);三组在0.25、0.5、1.0、2.0、3.0kHz这4个频率点的DPOAE幅值比较差异无统计学意义(P>0.05);A组与B组在4.0、6.0、8.0这3个频率点的DPOAE幅值与C组比较差异有统计学意义(P<0.05);A组在8.0这个频率点的DPOAE幅值与B组比较差异有统计学意义(P<0.05)。结论 DPOAE检查可对患者听力状态进行评估,具有较高的听力异常检出率,可用于临床判断ED患者预后。Objective To analyze the examination results of distortion product otoacoustic emission (DPOAE) in patients with explosive deafness (ED) and different prognosis.Methods A total of 66 patients with ED admitted to the hospital were enrolled as the research objects between January 2020 and January 2022.According to different prognosis,they were divided into group A (healing,n=14),group B (markedly effective,n=27) and group C (noneffective,n=25).The examination results of DPOAE among the three groups were compared.Results All the 66 patients (132 ears) underwent pure tone audiometry and DPOAE examination.In the 132 ears,there were 37 ears with abnormal pure tone hearing threshold and 52 ears with abnormal DPOAE.The detection rate of abnormal hearing in DPOAE examination was significantly higher than that in pure tone audiometry (39.39% vs 28.03%) (χ^(2)=4.316,P=0.038).There was no significant difference in DPOAE amplitudes under 0.25,0.5,1.0,2.0 and 3.0 kHz among the three groups (P>0.05).Compared with group C,differences in DPOAE amplitudes under 4.0,6.0 and 8.0 kHz in group A and group B were statistically significant (P<0.05).There were significant differences in DPOAE amplitude under 8.0 kHz between group A and group B (P<0.05).Conclusion DPOAE examination can evaluate hearing status of patients,with high detection rate of hearing abnormalities.And it can be applied to clinically determine the prognosis of ED patients.
