耳鸣患者瞬态诱发耳声发射的对侧抑制效应观察

摘　　要：目的探讨耳鸣患者在瞬态诱发耳声发射(TEOAE)的对侧抑制中的表现。方法对136例耳鸣患者收集其基本资料及耳鸣残障量表(tinnitus handicap inventory,THI)评估,并进行TEOAE的对侧抑制检测。同时,选取40名正常受试者进行对侧抑制TEOAE检查作为对照组进行对比。分析耳鸣组和对照组总体抑制(overall suppression,OS)值间的差异,及耳鸣患者的性别、年龄、侧别、患病时间和THI分级对其对影响。结果耳鸣组与对照组间OS值存在显著性差异(Z=2.88,P=0.004),性别、年龄、患病时间和THI分级对OS值无明显影响(P>0.05)。结论耳鸣患者的TEOAE对侧抑制值有所降低,存在内侧橄榄耳蜗束(medial olivocochlear,MOC)功能障碍。耳鸣患者的性别、年龄、侧别、患病时间和THI分级对TEOAE平均抑制影响不大。OBJECTIVE To explore the contralateral suppression of transient evoked otoacoustic emissions(TEOAE)in tinnitus patients.METHODS The basic data and tinnitus handicap inventory(THI)were collected from 136 patients with tinnitus,the contralateral suppression of TEOAE was measured.At the same time,40 normal subjects were selected as the control group and the contralateral suppression of TEOAE was measured.The differences of overall suppression(OS)values in the tinnitus control group and the effects of gender,age,side,duration of illness and THI grade were analyzed.RESULTS There was significant difference in the OS values between the tinnitus group and control group(Z=2.88,P=0.004).There were no significant effects of gender,age,duration of disease,and THI grade(P>0.05)on suppression values.CONCLUSION The contralateral suppression values of TEOAE were decreased in tinnitus group,and the dysfunction of MOC existed.The sex,age,side,duration of illness,and THI grade of tinnitus patients had no effect on the mean suppre ssion amplitude s of TEOAE.
