鼻腔鼻底异位牙及其所致囊肿的临床特征分析

摘　　要：目的分析鼻腔鼻底异位牙的临床特征。方法回顾经治疗的6例鼻腔硬腭区多生异位牙患者的临床资料,包括病史、鼻内镜检查及鼻窦CT等。结果6例患者的鼻腔鼻底异位牙主要发生在切牙孔及周围,3例(50%)以腥臭味为主要症状,4例异位牙向鼻腔生长,其中的2例(50%)合并有鼻塞,内镜表现为鼻底凸起的新生物。鼻窦CT检查能够清晰显示牙齿结构。有2例患者同时伴有上颌窦异位牙或含牙囊肿。6例异位牙均行内镜手术拔除,对于合并囊肿者同时切除全部囊皮或揭盖造袋。结论鼻腔异位牙临床表现的特异性不足,鼻内镜检查见鼻底质硬隆起时要考虑到异位牙的可能,CT检查能很好显示异位牙的结构及牙根位置。OBJECTIVE To analyze the clinical characteristics of ectopic teeth in the incisive foramen area of nasal cavity.METHODS Review the clinical data of six patients with ectopic supernumerary teeth in hard palate area,including medical history,nasal endoscopy and paranasal sinus CT.RESULTS In 6 patients,ectopic teeth in nasal and nasal floor mainly occurred around incisors,among which3 cases(50%)were characterized by foul smelling,4 cases with ectopic teeth growing into nasal cavity,2 cases(50%)with nasal obstruction and endoscopic manifestations were new organisms with nasal floor protrusion.CT scan can clearly show the structure of the structure of ectopic teeth.There were 2 cases(33%)with concurrent ectopic teeth or dentigerous cyst in maxillaty sinus.Ectopic teeth in 6 patients with were removed by endoscopic surgery,and the cystic membrane were also removed simultaneously or treat with uncover method for the patients’combined cysts.CONCLUSION The clinical manifestations of ectopic teeth in the nasal cavity are insufficient.During nasal endoscopy,ectopic teeth should be considered when seeing the hard bulge from the bottom of the nasal cavity.Moreover,the CT scan is a superior method that can clearly show the ectopic tooth structure and tooth root position.
