摘　　要：目的分析慢性鼻窦炎伴鼻息肉(chronic rhinosinusitis with nasal polyps,CRSwNP)伴发下气道炎症不同状态的肺功能,探讨上下呼吸道共病机制。方法回顾性分析2017年10月~2020年10月遂宁市中心医院耳鼻咽喉科79例CRSwNP患者的肺功能,其中无气道高反应(non-airway hyperresponsiveness,NAHR)组34例、无症状气道高反应(asymptomatic airway hyperresponsiveness,AAHR)组30例、支气管哮喘(bronchial asthma,BA)组15例,分析三组患者Lund-Mackay评分,Lund-Kennedy评分、外周血及鼻息肉嗜酸细胞(eosinophil,Eos)计数及其与肺功能的相关性。结果NAHR组、AAHR组及BA组肺功能呈递减趋势,组间有统计学差异(P<0.05),尤其是小气道功能。三组患者Lund-Mackay评分、Lund-Kenne dy评分及外周及息肉Eos计数呈递增趋势,组间存在统计学差异(P<0.05)。结论CRSwNP患者合并下气道不同炎症状态的肺功能损伤存在明显差异,临床医师也应注意CRSwNP患者的潜在小气道功能受损。OBJECTIVE To analyze the pulmonary function of chronic rhinosinusitis with nasal polyps(CRSwNP)accompanied with different airway inflammation,and to explore the co-disease mechanism between upper and lower respiratory tract.METHODS The pulmonary function of79 patients with CRSwNP in our department from October2017 to October 2020 were analyzed retrospectively,including34 patients in NAHR group,30 patients in AAHR group and15 patients in bronchial asthma(BA) group.The differences of Lund-Mackay score(LMS),Lund Kennedy score(LKS),eosinophil(Eos) count in peripheral blood and nasal polyps and their correlation with pulmonary function were analyzed.RESULTS Lung function decreased in NAHR,AAHR and BA groups,especially small airway function(P<0.05).Lundackay score,Lund-Kennedy score and Eos count in peripheral blood and polyps in the three groups showed an increasing trend,and there was significant statistical difference between the three groups(P<0.05).CONCLUSION There were significant statistical differences in pulmonary function injury in patients with CRSwNP combined with different inflammatory states of lower airway,clinicians should pay attention to the potential small airway dysfunction in CRSwNP patients.
