棘突终板角在腰椎融合术椎弓根置钉中的应用

摘　　要：目的探讨棘突终板角在腰椎融合术椎弓根置钉中的应用.方法选择2018年8月至2020年8月行开放式腰椎减压椎体间融合术(PLIF或TLIF)治疗的腰椎椎管狭窄或腰椎间盘突出症患者102例.观察组(54例)参照棘突终板角置钉,对照组(48例)采用常规AO技术置钉.结果两组患者术后CT扫描椎弓根螺钉与周围皮质关系及术后1个月VAS评分比较,差异无统计学意义(P>0.05);观察组患者术中置钉时间、透视次数少于对照组,差异有统计学意义(P<0.05);观察组钉板角小于对照组,差异有统计学意义(P<0.05).结论通过参照棘突终板角置钉与常规置钉具有同样的置钉准确性,却可以缩短置钉时间和减少透视次数,并获得更好矢状面置钉角度.Objective To explore the clinical application of spinous process-endplate angle in pedicle screw placement in lumbar fusion.Methods A total of 102 patients who underwent posterior lumbar interbody fusion or transforaminal lumbar interbody fusion(PLIF or TLIF)surgery from August 2018 to August 2020 were enrolled in this study.The pedicle screws(54 cases)in observation group were inserted according the angle of the spinous process-endplate,while the control group(48 cases)using conventional AO technique.Results There were no significant difference in the relationship of the pedicle and screw in the postoperative CT scan and the VAS score on postoperative 1 month(P>0.05)between the two groups.Compared to control group,observation group had less operative time of pedicle screw placement and fluoroscopy frequency,and the difference was statistically significant(P<0.05).The angle between the screw and the endplate in the observation group was smaller than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared to conventional placement,pedicle screw placement by referring to the spinous process endplate angle has the same accuracy,but it can reduce the time of pedicle screw placement and fluoroscopy frequency,and obtain a better screw fixation angle on the sagittal position.
