摘　　要：进行性核上性麻痹基本病机是下元虚衰、风痰阻窍;病理因素涉及风、痰、瘀、虚;病性为本虚标实,风、痰、瘀为实为标,虚为本,下元虚衰贯穿本病的始终。本病分为发作期和缓解期。发作期多以风痰邪实为主,久病入络,络脉瘀阻;缓解期病情稳定,多考虑下元虚衰为主。针对不同病机,提出滋阴补阳、熄风化痰、祛瘀通络等治法。发作期以祛邪实为主,多选用全蝎、白花蛇、水蛭、胆南星、瓜蒌等搜风通络化痰之品,其中以动物药为重;缓解期以补虚固本为主,多用地黄饮子滋阴补阳,化痰开窍。The basic pathogenesis of progressive supranuclear palsy(PSP)is lower Yuan deficiency and Wind-Phlegm blocking the orifices.Its pathological factors involve Wind,Phlegm,Blood stasis,and deficiency.The nature of the disease is asthenia in origin and asthenia in superficiality,with Wind,Phlegm,and Blood stasis being the symptoms and deficiency being the origin.The lower Yuan deficiency runs through the disease from beginning to end.The disease can be divided into the attack period and the remission period.The attack period is mostly dominated by Wind-Phlegm evil excessiveness,chronic illness enters the collaterals and the obstruction of vessel and collaterals.In the remission period,the condition is stable,and the main factor is to consider the lower Yuan deficiency.According to different pathogenesis,the treatment methods such as nourishing Yin and Yang,extinguishing Wind and resolving Phlegm,removing Blood stasis,and Dredging collaterals are proposed.During the attack period,the main purpose is to dispel pathogenic substances,and durgs such as Quanxie(Scorpio),Baihuashe(Bungarus minimus),Shuizhi(Hirudo),Dannanxing(Rhizoma Arisaematis Cum Bile),Gualou(Fructus Trichosanthis),etc.are used to search the Wind,clear the collaterals and resolve Phlegm,among which animal medicines are the most important.During the remission period,the main purpose is to invigorate deficiency and consolidate the Yuan Qi,and Dihuang Yinzi is often used to nourish Yin and Yang,to resolve Phlegm and to open the orifices.
