一次性包皮环切缝合器治疗包茎和包皮过长的临床实践

摘　　要：目的探讨一次性包皮环切缝合器治疗包茎和包皮过长的有效性与安全性。方法选取2017年5月至2021年2月山西省运城市中心医院诊治的420例包茎或包皮过长患者作为研究对象,其中185例患者采用传统的包皮环切术(A组),235例患者采用一次性包皮环切缝合器手术治疗(B组)。比较两组手术时间、术中出血量、有无系带保留偏短(<0.8cm)、术中疼痛评分、术后疼痛评分、术后疼痛持续时间,比较术后继发出血、切口裂开、淋巴水肿、切口感染等围手术期并发症发生率,比较术后患者外观满意度。结果B组手术时间短于A组,术中出血量少于A组,术中疼痛评分低于A组,系带保留偏短发生率高于A组,差异具有统计学意义(P<0.05)。两组术后疼痛评分、术后疼痛持续时间比较,差异无统计学意义(P>0.05)。A组术后继发出血、切口裂开发生率均高于B组,差异具有统计学意义(P<0.05)。两组术后淋巴水肿、术后切口感染发生率比较,差异无统计学意义(P>0.05)。B组术后外观满意度显著高于A组,差异具有统计学意义(P<0.05)。结论一次性包皮环切缝合器较传统的包皮环切术更简单易行,学习曲线短,技术容易掌握,手术时间短,术中出血量少,术后疼痛及外观满意度均有明显优势。Objective To investigate the effectiveness and safety of disposable circumcision suture device(DCSD)for phimosis and redundant prepuce.Methods 420 patients with phimosis or redundant prepuce diagnosed and treated in Yuncheng Central Hospital of Shanxi Province from May 2017 to February 2021 were retrospectively analyzed.185 of them underwent traditional circumcision(Group A),while the rest 235 patients received surgical treatment with Langhe brand DCSD(Group B).The two groups were compared in terms of operation time,intraoperative blood loss,short frenulum of prepuce(0.8cm),intraoperative pain score,postoperative pain score,duration of post-operative pain,post-operative secondary bleeding,post-operative disruption of wound,lymphedema,infection of incisional wound,perioperative complications,and patient′s postoperative appearance satisfaction.Results The operation time of group B was shorter than that of group A,the intraoperative blood loss was less than that of group A,the intra-operative pain score was lower than that of group A,and the incidence of short frenulum of prepuce was higher than that of group A,while the differences were all statistically significant(P<0.05).There was no significant difference in postoperative pain score and duration of post-operative pain between the two groups(P>0.05).The incidence of post-operative secondary bleeding and post-operative disruption of wound in group A was higher than those in group B(P<0.05).There was no significant difference in the incidence of lymphedema and infection of incisional wound between the two groups(P>0.05).The patient′s satisfaction on postoperative appearance in group B was significantly higher than that in group A(P<0.05).Conclusions Compared with traditional circumcision,DCSD is simpler and easier to operate,requires a shorter learning curve,more understandable technology and shorter operation time,and shows many obvious advantages in intraoperative bleeding loss,post-operative pain and appearance satisfaction.
