摘　　要：目的比较经尿道前列腺电切术(TURP)及经尿道前列腺等离子剜除术(TUPKEP)对良性前列腺增生(BPH)患者尿道功能和性功能的影响。方法选取2018年1月至2020年12月德阳市人民医院收治的84例BPH患者作为研究对象。随机抽样分为对照组(n=42,采用TURP治疗)和试验组(n=42,采用TUPKEP治疗)。记录两组手术时间、住院天数、腺体组织重量、导管留置时间;术前及术后3个月,比较两组国际前列腺症状评分法(IPSS)评分、残余尿量、最大尿流率、勃起功能指数;观察术后并发症发生情况。结果试验组手术时间和住院天数明显短于对照组,差异具有统计学意义(P<0.05)。两组腺体组织重量、导管留置时间比较,差异无统计学意义(P>0.05)。与术前比较,两组术后3个月勃起功能指数、最大尿流率均明显增高,且试验组优于对照组,差异具有统计学意义(P<0.05);与术前比较,两组术后3个月IPSS评分和残余尿量均明显减少,且试验组优于对照组,差异具有统计学意义(P<0.05)。试验组并发症发生率低于对照组,差异具有统计学意义(P<0.05)。结论BPH患者采用TUPKEP治疗的效果优于TURP,可减少手术时间,改善尿道功能和性功能,且安全性较高。Objective To compare the effects of transurethral resection of prostate(TURP)and transurethral plasma enucleation of prostate(TUPKEP)on urethral function and sexual function in patients with benign prostatic hyperplasia(BPH).Methods 84 patients with BPH admitted to the People′s Hospital of Deyang City from January 2018 to December 2020 were selected as the research objects,which were randomly divided into control group(n=42,treated with TURP)and experimental group(n=42,treated with TUPKEP).The operation time,hospitalization days,gland tissue weight and catheter retention time of the two groups were recorded.Before and 3 months after operation,the international prostate symptom score(IPSS),residual urine volume,maximum urinary flow rate and erectile function index were compared between the two groups.The postoperative complications were observed.Results The operation time and hospitalization days in experimental group were significantly shorter than those of the control group(P<0.05).There was no difference in gland tissue weight and catheter retention time between two groups(P>0.05).Compared with preoperative,the erectile function index and maximum urinary flow rate of the two groups were significantly higher,and the experimental group was better than the control group,the difference was statistically significant(P<0.05).Compared with the preoperative,the IPSS and residual urine volume of the two groups in 3 months after operation were significantly reduced,and the experimental group was better than the control group,the difference was statistically significant(P<0.05).The incidence of complications in experimental group was significantly lower than that of the control group(P<0.05).Conclusions Compared with TURP,TUPKEP is more effective in the treatment of BPH,which can reduce the operation time,improve the urethral function and sexual function of patients,and has high safety.
