羊膜移植与生长激素联合治疗对中重度宫腔粘连术后宫腔形态、子宫内膜厚度及再粘连率的影响

摘　　要：目的探讨羊膜移植与生长激素结合对中重度宫腔粘连术后宫腔形态、子宫内膜厚度及再粘连率的影响。方法选取2017年6月至2019年8月就诊于遵义医科大学附属妇幼保健院的92例中重度宫腔粘连术后患者作为研究对象,采用随机数字表法分为观察组和对照组,各46例。在常规治疗基础上,对照组采取生长激素治疗,观察组采取羊膜移植与生长激素联合治疗。统计并比较两组宫腔形态恢复情况、月经改善情况、治疗前后子宫内膜厚度及治疗后再粘连发生率。结果观察组治疗3个月经周期后宫腔形态恢复总有效率、月经改善率(93.48%、89.13%)较对照组(78.26%、71.74%)高(P<0.05);治疗1个月经周期、3个月经周期后观察组子宫内膜厚度高于对照组(P<0.05);观察组治疗后6个月再粘连发生率(15.22%)较对照组(34.78%)低(P<0.05)。结论采取羊膜移植与生长激素联合治疗中重度宫腔粘连术后患者,可改善宫腔形态及子宫内膜厚度,降低再粘连风险,效果显著。Objective To investigate the effect of amniotic membrane transplantation combined with growth hormone on uterine cavity morphology,endometrial thickness and readhesion rate after operation of moderate and severe intrauterine adhesion.Methods 92 patients with moderate and severe intrauterine adhesions after surgery in Maternal and Child Health Care Hospital Affiliated to Zunyi Medical University from June 2017 to August 2019 were selected as the research objects and divided into observation group and control group by random number table method,with 46 cases in each group.On the basis of conventional treatment,the control group was treated with growth hormone,and the observation group was treated with amniotic membrane transplantation combined with growth hormone.The uterine cavity morphology recovery,menstrual improvement,endometrial thickness before and after treatment,and the incidence of readhesion after treatment were statistically compared between the two groups.Results The total effective rate of uterine cavity morphology recovery and menstrual improvement rate(93.48%,89.13%)in the observation group after 3 menstrual cycles were higher than those in the control group(78.26%,71.74%)(P<0.05);After 1 menstrual cycle and 3 menstrual cycles,the endometrial thickness of the observation group was higher than that of the control group(P<0.05);the incidence of readhesion 6 months after treatment in the observation group(15.22%)was lower than that in the control group(34.78%)(P<0.05).Conclusions Amniotic membrane transplantation combined with growth hormone in the treatment of patients with moderate and severe intrauterine adhesions can improve the uterine cavity morphology and endometrial thickness,reduce the risk of readhesion,and has a significant effect.
