摘　　要：目的探讨二甲双胍联合达英-35辅助促排卵对多囊卵巢综合征(PCOS)患者的治疗效果。方法将2018年1月至2019年12月于长治市妇幼保健院就诊的102例PCOS患者作为观察组,根据治疗方式的不同分为观察Ⅰ组(n=52,行二甲双胍联合达英-35辅助促排卵治疗)和观察Ⅱ组(n=50,行单纯促排卵治疗),另选取50例同期体检健康女性作为对照组。比较观察组和对照组的临床指标;比较观察Ⅰ组和观察Ⅱ组治疗前后25-羟维生素D[25-(OH)D]、糖代谢指标、内分泌指标及临床症状,并评价上述两组的总体疗效及不良反应。结果与对照组相比,观察组治疗前体重指数、腰围、腰臀比、空腹血糖、胰岛素、胰岛素抵抗指数、甘油三酯、总胆固醇、低密度脂蛋白、睾酮及黄体生成素均显著升高,而高密度脂蛋白和25-(OH)D显著降低(P<0.05);与治疗前相比,观察组治疗后25-(OH)D、糖代谢、内分泌指标及临床症状均显著改善(P<0.05),与观察Ⅱ组相比,观察Ⅰ组疗效更显著(P<0.05)。结论采用二甲双胍联合达英-35辅助促排卵治疗育龄期PCOS患者能获得更好的临床疗效,且不会增加用药风险。Objective To investigate the therapeutic effect of metformin combined with Diane-35 in the treatment of polycystic ovary syndrome(PCOS).Methods A total of 102 PCOS patients treated in Changzhi Maternal and Child Health Care Hospital from January 2018 to December 2019 were selected as the observation group.According to the different treatment methods,they were divided into observation groupⅠ(n=52,treated with metformin combined with Diane-35 assisted ovulation induction)and observation groupⅡ(n=50,treated with ovulation induction alone).Another 50 healthy women were selected as the control group.The clinical indexes of observation group and control group were compared.25-hydroxyvitamin D[25-(OH)D],glucose metabolism,endocrine indexes and clinical symptoms before and after treatment were compared between observation groupⅠand observation groupⅡ,and the overall efficacy and adverse reactions of the two groups were evaluated.Results Compared with the control group,body mass index,waist circumference,waist-to-hip ratio,fasting blood glucose,insulin,insulin resistance index,triglyceride,total cholesterol,low density lipoprotein,testosterone and luteinizing hormone in observation group before treatment were significantly increased,while high density lipoprotein and 25-(OH)D were significantly decreased(P<0.05);compared with before treatment,25-(OH)D,glucose metabolism,endocrine indexes and clinical symptoms of observation group were significantly improved after treatment(P<0.05),and the efficacy in observation groupⅠwas more significant than that in observation groupⅡ(P<0.05).Conclusions Metformin combined with Diane-35 for ovulation induction therapy in PCOS patients of reproductive age can achieve better clinical efficacy without increasing the risk of medication.
