血清妊娠相关血浆蛋白-A、白介素-8与输卵管妊娠患者保守治疗效果的相关性

摘　　要：目的观察血清妊娠相关血浆蛋白-A(PAPP-A)、白介素-8(IL-8)与输卵管妊娠患者保守治疗效果的相关性。方法回顾性分析儋州市人民医院2018年7月至2021年7月收治的87例行保守治疗的输卵管妊娠患者的临床资料,根据保守治疗效果将患者分为治疗失败组(n=14)与治疗成功组(n=73)。记录并比较两组血清PAPP-A、IL-8水平,采用Logistic回归分析血清PAPP-A、IL-8水平与输卵管妊娠患者保守治疗效果的相关性,分析血清PAPP-A、IL-8水平对输卵管妊娠患者保守治疗效果的预测效能。结果多元Logistic回归分析结果显示,血清IL-8表达上调是输卵管妊娠患者保守治疗失败的危险因素(OR>1,P<0.05);血清PAPP-A表达上调是输卵管妊娠患者保守治疗失败的保护因素(OR<1,P<0.05)。绘制受试者工作特征(ROC)曲线发现,血清PAPP-A、IL-8水平单项及联合预测输卵管妊娠患者保守治疗效果的曲线下面积(AUC)均>0.70,有中等预测价值。结论血清IL-8表达上调、PAPP-A表达下调与输卵管妊娠患者保守治疗失败有关,可用于预测患者保守治疗失败的发生风险。Objective To observe the correlation between serum pregnancy associated plasma protein-A(PAPP-A),interleukin-8(IL-8)and effect of conservative treatment in patients with tubal pregnancy.Methods The clinical data of 87 patients with tubal pregnancy treated in Danzhou People′s Hospital from July 2018 to July 2021 were retrospectively analyzed.The patients were divided into treatment failure group(n=14)and treatment success group(n=73)according to the effect of conservative treatment.The levels of serum PAPP-A and IL-8 were recorded and compared between the two groups.Logistic regression analysis was used to test the correlation between the levels of serum PAPP-A and IL-8 and the effect of conservative treatment in patients with tubal pregnancy,and the predictive efficacy of serum PAPP-A and IL-8 levels on effect of conservative treatment in patients with tubal pregnancy was analyzed.Results The results of multiple Logistic regression analysis showed that the up-regulation of serum IL-8 expression was a risk factor for failure of conservative treatment in patients with tubal pregnancy(OR>1,P<0.05);the up-regulation of serum PAPP-A expression was a protective factor for the failure of conservative treatment in patients with tubal pregnancy(OR<1,P<0.05).The receiver operating characteristic(ROC)curve showed that area under the curve(AUC)of single and combined prediction of serum PAPP-A and IL-8 levels on effect of conservative treatment in patients with tubal pregnancy were both>0.70,which had medium predictive value.Conclusions The up-regulated expression of serum IL-8 and down-regulated expression of PAPP-A are related to the failure of conservative treatment in patients with tubal pregnancy,which can be used to predict the risk of failure of conservative treatment.
