男性糖尿病患者血清性激素结合球蛋白水平与骨矿物质密度的相关性研究

摘  要:目的:探讨男性糖尿病患者血清性激素结合球蛋白水平与骨矿物质密度的相关性。方法:本次试验选取2019年1月—2021年1月南方科技大学医院收治的86例男性糖尿病患者作为研究组,并选取同时期收治的46例男性健康体检者作为对照组,对比两组受试者基础资料和血清性激素结合球蛋白水平,分析与骨矿物质密度的相关性。结果:研究组BMI、FPG、FINS、UA及HOMA-IR水平高于对照组,差异有统计学意义(t=4.917、12.837、10.879、2.196、11.620,P<0.05);研究组FSH、LH、T、E及E2/T水平高于对照组,差异有统计学意义(t=6.319、14.290、3.066、14.020、12.690,P<0.050.05);腰椎L、L、L、股骨颈、大转子、华氏三角及髋部整体骨密度与年龄呈负相关,(r=-0.310、-0.285、-0.306、-0.391、-0.283、-0.247、-0.365,P<0.05);血清E、血清FT与全髋骨密度及腰椎L-L、25-羟维生素D呈正相关(P<0.05),血清E、血清FT与尿NTX及BALP呈负相关(P<0.05)。结论:男性糖尿病患者代谢变化的表征指标为血清性激素结合球蛋白水平,与骨矿物质密度具有相关性。Objective: To investigate the correlation between serum sex hormone binding globulin level and bone mineral density in male diabetic patients. Methods: A total of 86 male diabetic patients admitted to the hospital from January 2019 to January 2021 were selected as the research group, and 46 healthy male patients admitted during the same period were selected as the control group. The basic data and serum sex hormone-binding globulin levels were compared between the two groups, and the correlation with bone mineral density was analyzed. Results: The levels of BMI, FPG, FINS, UA and HOMA-IR in the study group were higher than those in the control group, and the difference was statistically significant(t=4.917, 12.837, 10.879, 2.196, 11.620, P<0.05). The levels of FSH, LH, T, Eand E/T in the study group were higher than those in the control group, and the difference was statistically significant(t=6.319, 14.290, 3.066, 14.020, 12.690, P<0.05). Lumbar vertebrae L, L, L, femoral neck, greater trochanter, Fahrenheit triangle and overall hip BMD were negatively correlated with age(r=-0.310,-0.285,-0.306,-0.391,-0.283,-0.247,-0.365, P<0.05). Serum E2 and serum FT were positively correlated with total hip BMD and lumbar spine L-Land 25-hydroxyvitamin D(P<0.05). Serum Eand serum FT were negatively correlated with urinary NTX and BALP(P<0.05). Conclusion: Metabolic changes in male diabetic patients are characterized by serum sex hormone-binding globulin levels, which are correlated with bone mineral density.
