摘 要:目的:探究血清同型半胱氨酸(Hcy)、游离雌三醇(FE3)、白介素12 (IL-12)在妊娠期高血压患者(HDP)中的表达及临床意义。方法:选取2018年5月—2020年5月东莞东华医院妇产科收治的72例HDP患者为观察组(包括妊娠期高血压组、子痫前期组、重度子痫前期组)及31例妊娠晚期正常孕妇为对照组(正常妊娠组),均进行相关生化指标、血清Hcy、FE3、IL-12指标检测,分析Hcy、FE3、IL-12水平与HDP分级及血小板计数(PLT)、纤维蛋白原(FIB)指标相关性。结果:重度子痫前期组凝血酶原时间(PT)、凝血活酶时间(APTT)水平均显著低于正常妊娠组,血小板分布宽度(PDW)水平显著高于正常妊娠组、妊娠期高血压组,差异有统计学意义(F=1.748、3.633、4.827,P<0.05),随HDP病症分级不断增加,观察组各亚组纤维蛋白原(FIB)、Hcy、IL-12水平显著升高,PLT、FE3水平显著下降,对照组与观察组之间,差异有统计学意义(F=8.809、9.702、37.502、8.812、2.817,P<0.05);HDP患者血清Hcy、IL-12水平与PLT水平显著负相关,与FIB水平、HDP分级显著正相关。HDP患者血清FE3水平与PLT水平显著正相关,与FIB水平、HDP分级显著负相关。结论:血清Hcy、FE3、IL-12水平可反映HDP状态,检测血清Hcy、FE3、IL-12有助于衡量HDP患者病情及凝血功能变化。Objective: To investigate the expression and clinical significance of serum homocysteine(Hcy), free estradiol(FE3) and interleukin-12(IL-12) in patients with gestational hypertension(HDP). Methods: A total of 72 HDP patients admitted to the hospital from May 2018 to May 2020 were selected as the observation group(including gestational hypertension group, preeclampsia group, and severe preeclampsia group) and 31 normal pregnant women in the third trimester as the control group(normal pregnancy group). All patients were tested for related biochemical indicators, serum Hcy, FE3, and IL-12 indicators. The correlation of Hcy, FE3, IL-12 levels with HDP grade, platelet count(PLT) and fibrinogen(FIB) indexes was analyzed. Results: The levels of PT and APTT in the severe preeclampsia group were significantly lower than those in the normal pregnancy group, and the PDW level was significantly higher than that in the normal pregnancy group and the gestational hypertension group, with statistically significant differences(F=1.748、3.633、4.827,P<0.05). With the continuous increase of HDP disease classification, the levels of FIB, Hcy, and IL-12 in each subgroup of the observation group were significantly increased, while the levels of PLT and FE3 were significantly decreased, and the difference between the control group and the observation group was statistically significant(F=8.809、9.702、37.502、8.812、2.817,P<0.05). Serum Hcy and IL-12 levels in HDP patients were significantly negatively correlated with PLT levels, and significantly positively correlated with FIB levels and HDP grades. Serum FE3 level in HDP patients was significantly positively correlated with PLT level, and significantly negatively correlated with FIB level and HDP grade. Conclusion: Serum Hcy, FE3, and IL-12 levels can reflect HDP status. Detection of serum Hcy, FE3, and IL-12 is helpful to measure the condition and changes of coagulation function in HDP patients.
