摘　　要：目的:应用应变成像技术(SI)评估阵发性房颤(PAF)患者经导管射频消融术(RFCA)前后右心房功能的变化。方法:选取2019年5月~2021年7月于中国科学技术大学附属第一医院行RFCA治疗的PAF患者36例。同期选择性别、年龄与PAF组相匹配的健康志愿者36例作为对照组。患者术前及术后3个月行常规经胸超声心动图检查,存储图像,应用SI技术描记获取右心房应变-时间曲线。测量储备阶段、管道阶段及泵阶段的右心房整体纵向应变值,对术前与对照组,术后3个月与术前右心房各阶段整体纵向应变值间的差异性进行比较。结果:PAF组LVIDd、LA、RALR、RAUD均高于对照组(P<0.05),LVEF、Sct_AC绝对值、Scd_AC绝对值、Sr_AC均低于对照组(P<0.05);PAF组术后3个月LA、RALR、RAUD均较术前降低(P<0.05),Sr_AC、Scd_AC绝对值均较术前升高(P<0.05)。结论:SI结合常规超声可以更全面地反映PAF患者RFCA治疗对右心房结构和功能产生的影响。Objective:To observe the outcomes of strain imaging(SI) in evaluation of the right atrial function in patients with paroxysmal atrial fibrillation(PAF) undergoing radiofrequency catheter ablation(RFCA).Methods:Thirty-six PAF cases undergone RFCA in the First Affiliated Hospital of USTC were included from May 2019 to July 2021,and another 36 healthy volunteers matching the gender and age of the RFCA group were recruited as normal controls.All PAF patients underwent routine transthoracic echocardiography before and 3 months after surgery, with the images stored.Then SI technology was used to obtain the strain-time curve of the right atrium.The overall longitudinal strain values of the right atrium were respectively measured in the reservoir, conduit and contraction phase, and compared for the differences with the normal controls before operation and within the RFCA group at each stage 3 months after operation.Results:LVIDd, LA, RALR and RAUD were higher in PAF group than in control group(P<0.05),yet LVEF,absolute values of Sct_AC,Scd_AC and Sr_AC were all lower in PAF group than in control group(all P<0.05).Patients in the PAF group had decreased LA,RALR and RAUD in 3 months after operation compared to pre-operation(P<0.05),and increased absolute values of Sr_AC and Scd_AC compared to those before operation(P<0.05).Conclusion:Strain imaging combined with conventional ultrasonography can all round demonstrate the changes in the structure and function of the right atrium following radiofrequency ablation of atrial fibrillation.
