摘 要:目的:观察IgA肾病患者扁桃体淋巴细胞亚群分布特点,探讨生发中心及滤泡间区淋巴细胞变化与蛋白尿的相关性。方法:IgA肾病行扁桃体切除术患者31例,采用免疫组化方法检测生发中心及滤泡间区的CD_(4)、CD_(19)、CD_(20)、CD_(21)及IgA1的标记细胞数,使用单因素分析、线性回归及曲线拟合对尿蛋白与淋巴细胞亚群进行分析。结果:24 h尿蛋白与生发中心的CD^(+)_(21)淋巴细胞计数呈显著正相关。生发中心CD^(+)_(21)淋巴细胞计数及滤泡间区CD^(+)_(19)淋巴细胞计数与尿蛋白水平正相关,生发中心CD^(+)_(19)淋巴细胞计数与24 h尿蛋白呈n形曲线关系。结论:IgA肾病患者扁桃体生发中心及滤泡间区的淋巴细胞分布与尿蛋白水平存在明显相关性,可能与IgA肾病的病情进展密切相关。Objective:To explore the correlation between the changes of lymphocytes in tonsil germinal center and interfollicular area and proteinuria in patients with IgA nephropathy.Methods:Thirty-one IgA nephropathy patients who underwent tonsillectomy were enrolled in this study.Immunohistochemical analysis was used to detect the number of CD_(4),CD_(19),CD_(20),CD_(21),and IgA1 labeled cells in the germinal center and interfollicular area.Linear regression and curve fitting were used to analyze the correlation between 24-hour urinary protein level and the distribution of lymphocyte subsets in tonsil tissue.Results:The correlation analysis showed that 24-hour urinary protein was significantly positively correlated with the CD^(+)_(21)lymphocytes of the tonsil germinal center.CD^(+)_(21)lymphocytes in the germinal center and CD^(+)_(19)lymphocytes in the interfollicular area were positively correlated with urinary protein levels.There was an n-shaped relationship between CD^(+)_(19)lymphocytes and 24-hour urine protein in the germinal center.Conclusion:There are significant correlations between lymphocyte subsets distribution in the germinal center and interfollicular area of the tonsil and the level of proteinuria in patients with IgA nephropathy,suggesting that may be closely related to the progression of IgA nephropathy.
