富血小板血浆干预大鼠骨性关节炎进程的实验研究

摘　　要：目的研究富血小板血浆干预大鼠骨性关节炎(Osteoarthritis,OA)进程的实验。方法选取32只SD(Sprague-Dawley)大鼠,随机分为4组,每组8只。正常组(CK)不做干预,假手术组(Sham)切开膝关节腔,关节炎组(OA)改良Hulth法造模,PRP组采用改良Hulth法造模后予以富血小板血浆(platelet-rich plasm,PRP)关节腔内注射。6周后,采用X线摄片观察关节骨质改变并进行K-L(Kellgren-Lawrence)分级,HE(苏木精-伊红)染色观察软骨及软骨下骨病理学改变并进行Mankin评分,ELISA法检测关节液中HA(透明质酸)、COMP(软骨寡聚基质蛋白)、CTX-I(I型胶原C端肽)含量的变化,得到的数据用统计学方法对比分析。结果X线摄片Kellgren-Lawrence分级以及HE染色Mankin评分,关节炎组对比正常组有显著差异(P<0.05);关节炎组对比PRP组有显著差异(P<0.05)。ELISA检测关节液中HA、COMP、CTX-I的含量并做统计学分析,关节炎组与正常组对比有显著差异(P<0.05);PRP组与关节炎组对比有显著差异(P<0.05)。结论富血小板血浆通过干预骨、软骨及滑膜的代谢影响着骨性关节炎的进程。Objective To investigate how platelet-rich plasma affects the progression of Osteoarthritis(OA)in rats.Methods A total of 32 SD(Sprague-Dawley)rats were randomly divided into 4 groups of 8 rats each.No intervention was performed in the normal group(CK).The sham group(Sham)cut the knee cavity.A modified Hulth model method for arthritis group(OA).The PRP group was injected platelet u-rich plasm(PRP)with modified Hulth method.After 6 weeks using X-ray observe joint bone changes and Kellgren-Lawrence grading,HE(hematoxylin-eosin)staining observation cartilage and subcartilage bone pathology changes and Mankin score,ELISA joint solution to detect HA(hyaluronic acid),COMP(Cartilage oligomeric matrix protein),CTX-I(C-terminal telopeptide of collagen I)content,the data by statistical methods.ResultsKellgren-Lawrence and HE staining showed normal group(P<0.05)and PRP group(P<0.05).ELISA measured the contents of HA,COMP and CTX-I in joint fluid and statistical analysis showed significant differences between arthritis and normal(P<0.05),PRP and arthritis(P<0.05).Conclusion Platelet-rich plasma affects the course of OA by intervening in the metabolism of bone,cartilage and synovium.
