摘 要:目的:分析围手术期营养管理在脊髓损伤患者中的应用效果。方法:选取2018年1月—12月南京大学医学院附属鼓楼医院脊柱外科收治的脊髓损伤患者30例为对照组,采用常规饮食管理。给予饮食指导,发放饮食指导手册;2019年1月—12月南京大学医学院附属鼓楼医院脊柱外科收治的30例脊髓损伤患者为干预组,干预组在对照组的基础上采用围手术期营养管理。即:制定综合营养管理模式,成立营养管理小组,制定个性化饮食计划,肠内联合肠外营养,再加上综合肠道管理,关注患者血液检测指标。比较两组患者围术期的总蛋白、白蛋白、血红蛋白、低钠血症、并发症发生率。结果:两组患者围术期的总蛋白、白蛋白、血红蛋白比较,差异有统计学意义(P<0.05);干预组低钠血症的发生率低于对照组,差异有统计学意义(P<0.05);两组患者口服康全甘后并发症发生率比较,差异无统计学意义(P>0.05)。结论:给予脊髓损伤患者围手术期营养管理,有利于促进患者恢复,提高手术效果。Objective To analyze the application effect of perioperative nutritional management in patients with spinal cord injury.Methods We admitted 30 patients with spinal cord injury as the control group from January to December 2018 in our department,who had adopted the routine diet guidance and diet guidance handbook.And the 30 patients with spinal cord injury from January to December 2019 were taken as the intervention group.Comprehensive intervention measures included:established the nutrition management group,developed the personalized diet plan,combined enteral nutrition with parenteral nutrition and integrated intestinal management.The intervention group was formulated the comprehensive nutrition management mode on the basis of the control group.We tested the blood indicators of patients during the perioperative period.Results There were a significant difference in the total protein,albumin and hemoglobin of the two groups(P<0.05).The incidence of hyponatremia in the intervention group was lower than that in the control group(P<0.05).There were no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion It is beneficial to promote patients recover and better tolerance of surgery to use the nutritional management in the perioperative period of spinal cord injury.
