摘　　要：目的 剖析以DRG为基础的恶性肿瘤病例分组效果与住院费用结构,为按DRG支付的推广提供科学依据。方法 使用变异系数、总体变异减低系数来评价恶性肿瘤DRG分组效果,采用结构变动度法分析其DRG组之间的结构变动情况。结果 恶性肿瘤DRG组的总体变异减低系数为92.57%,RE19与RU14区间的结构变动最大。结论 恶性肿瘤DRG病例分组效果显著,体现了按DRG付费的初衷;为保障医务人员的合理报酬,需重视反映医务人员技术劳务价值的成本费用。Objective To analyze the effect of grouping malignant tumor cases and the structure of hospitalization expenses based DRG,and provide a scientific basis for the promotion of DRG payment.Methods The coefficient of variation and the coefficient of overall reduction of variation were used to evaluate the effect of DRG grouping of malignant tumors,and the structural change method was used to analyze the structural changes between the DRG groups.Results The coefficient of overall reduction of variation of the DRG group of malignant tumors was 92.57%;the structural change between RE19 and RU14 was the largest.Conclusion The effect of grouping DRG cases of malignant tumors was significant,which reflects the original intention of DRG payment.To ensure the reasonable remuneration of medical staff,it is necessary to pay attention to the cost that reflects the value of medical staff’s technical labor service.
